Chest tightness, hand numbness, and gastrointestinal discomfort?
Hello, doctor.
Recently, I have been experiencing chest tightness accompanied by numbness and weakness in my arms (often the left arm), gastrointestinal discomfort (diarrhea), and a sensation of something stuck in my throat.
At night, just as I'm about to fall asleep, my left hand twitches, causing me to wake up suddenly, which affects my sleep.
I also frequently experience dizziness, neck and shoulder pain, coccyx pain (unable to sit for long periods), and easy fatigue.
I have consulted with gastroenterology, endocrinology, and cardiology, and no issues were found.
The gastroenterologist performed a stool occult blood test and prescribed medication.
The endocrinologist conducted an ultrasound and blood tests to confirm there are no thyroid abnormalities, and I had an X-ray and an electrocardiogram for my heart, which also showed no abnormalities.
I would like to inquire if there are any other specialties I could seek help from.
Thank you.

More Info
Based on the symptoms you described—chest tightness, numbness and weakness in the left arm, gastrointestinal discomfort, a sensation of a lump in the throat, sleep disturbances, dizziness, neck and tailbone pain, and fatigue—it's understandable that you are seeking further evaluation. While you've already consulted specialists in gastroenterology, metabolism, and cardiology without finding any significant issues, it's important to consider a comprehensive approach to your symptoms.
Possible Causes and Considerations
1. Anxiety and Stress: One of the most common causes of chest tightness and numbness in the extremities is anxiety. Anxiety can manifest physically, leading to symptoms such as chest discomfort, tingling or numbness in the hands, and gastrointestinal issues like diarrhea. Given the range of symptoms, it might be worthwhile to explore whether anxiety or stress could be contributing to your condition. Cognitive-behavioral therapy (CBT) or relaxation techniques may be beneficial.
2. Neurological Evaluation: Since you are experiencing numbness and weakness in your left arm, it may be prudent to consult a neurologist. Conditions such as carpal tunnel syndrome, cervical radiculopathy, or even peripheral neuropathy could be responsible for your symptoms. A thorough neurological examination, possibly including imaging studies like an MRI of the cervical spine or nerve conduction studies, could provide insights.
3. Musculoskeletal Issues: Given your complaints of neck and tailbone pain, musculoskeletal issues could also be a factor. Conditions such as cervical spondylosis or muscle strain can lead to referred pain and discomfort in the chest and arms. A consultation with a physical therapist or an orthopedic specialist may help in assessing and managing these symptoms.
4. Gastroesophageal Reflux Disease (GERD): Your gastrointestinal symptoms, particularly the sensation of a lump in your throat and abdominal discomfort, could suggest GERD. This condition can cause chest tightness and mimic cardiac symptoms. A gastroenterologist may recommend further evaluation, such as an upper endoscopy or pH monitoring, to assess for reflux.
5. Sleep Disorders: The episodes of your left hand shaking as you are falling asleep could indicate a sleep disorder, such as sleep apnea or periodic limb movement disorder. A sleep study could help diagnose any underlying sleep issues that may be contributing to your fatigue and discomfort.
6. Holistic Approach: Sometimes, a multidisciplinary approach is necessary. Consider consulting a holistic or integrative medicine specialist who can look at your symptoms from a broader perspective, including lifestyle factors, nutrition, and overall wellness.
Next Steps
- Follow-Up Appointments: Schedule follow-up appointments with the specialists mentioned above, particularly a neurologist and possibly a psychologist or psychiatrist for anxiety management.
- Lifestyle Modifications: Implement lifestyle changes that may alleviate some symptoms, such as regular exercise, a balanced diet, stress management techniques, and ensuring adequate sleep hygiene.
- Documentation: Keep a detailed symptom diary, noting when symptoms occur, their duration, and any potential triggers. This information can be invaluable for your healthcare providers in diagnosing and managing your condition.
Conclusion
While your previous evaluations have not indicated any serious underlying conditions, the persistence and variety of your symptoms warrant further investigation. A multidisciplinary approach may provide the best chance of identifying the root cause of your discomfort and developing an effective treatment plan. Don't hesitate to advocate for yourself and seek additional opinions if necessary. Your health and well-being are paramount, and addressing these symptoms holistically may lead to significant improvements in your quality of life.
