Glans problems?
Hello Doctor: Since last year, I have been experiencing issues with my glans.
It started with small red spots, progressed to localized redness, and eventually ulcers.
I have visited clinics for medication treatment, and although the condition improves after each treatment, the glans easily develops localized redness again after a while.
There is no itching or pain, and during this time, I have had no sexual contact.
Could you please explain what might be causing the glans to become easily red? I wash the glans daily with room temperature water while retracting the foreskin and do not use any cleaning products excessively.
What can I do to prevent this recurring localized redness?

Hello: Glans ulcers are not a normal phenomenon.
It is recommended to visit a major hospital's urology or infectious disease department to rule out certain infectious diseases.
If the cause remains unclear, diseases related to rheumatology and immunology should also be considered.
Additionally, maintaining cleanliness around the genital area is very important.

Persistent redness of the glans, or glansitis, can be a frustrating and concerning issue. It is important to understand that while the symptoms you describe—redness, small red spots, and ulcers—can be alarming, they are often manageable with the right approach.
There are several potential causes for persistent redness of the glans, including:
1. Infections: Fungal infections, particularly candidiasis (yeast infections), are common culprits. Bacterial infections can also occur, although they are less frequent. These infections can lead to inflammation and redness.
2. Dermatitis: Contact dermatitis, which can be caused by irritants or allergens, may lead to redness. This could be due to soaps, lotions, or even laundry detergents that come into contact with the genital area.
3. Poor Hygiene: While you mentioned that you clean the area regularly, inadequate cleaning can lead to the accumulation of smegma, which can irritate the glans. Conversely, over-cleaning with harsh soaps can strip the skin of its natural oils, leading to irritation.
4. Skin Conditions: Conditions such as psoriasis or eczema can also affect the genital area, leading to redness and irritation.
5. Phimosis or Balanitis: If the foreskin is too tight (phimosis), it can lead to inflammation of the glans (balanitis), causing redness and discomfort.
6. Sexually Transmitted Infections (STIs): Although you mentioned no recent sexual contact, it is still important to consider STIs as a potential cause, especially if there were any past exposures.
Given that you have experienced recurrent episodes of redness, it is advisable to consult with a healthcare provider, preferably a urologist or dermatologist, who can conduct a thorough examination and possibly perform tests to determine the underlying cause.
Recommendations for Management:
1. Gentle Cleaning: Continue to clean the area gently with warm water. Avoid soaps or cleansers that may irritate the skin. If you must use soap, opt for a mild, fragrance-free product.
2. Avoid Irritants: Be cautious with any products that come into contact with the genital area. This includes lotions, powders, and even certain fabrics that may cause irritation.
3. Moisturizing: If dryness is an issue, consider using a gentle, hypoallergenic moisturizer to keep the skin hydrated.
4. Avoid Tight Clothing: Wear loose-fitting underwear and pants to reduce friction and irritation in the area.
5. Monitor Symptoms: Keep track of any changes in symptoms, including the appearance of new spots, changes in color, or any discharge. This information can be helpful for your healthcare provider.
6. Follow-Up Care: Since you have had multiple treatments with temporary relief, it may be beneficial to seek a second opinion or further evaluation to explore other treatment options, including topical antifungals or corticosteroids if an inflammatory condition is suspected.
7. Consider Dietary Factors: While not directly related, some individuals find that certain foods can exacerbate skin conditions. Keeping a food diary may help identify any potential triggers.
In conclusion, persistent glans redness can arise from various causes, and while it may not be painful or itchy, it is essential to address it to prevent further complications. A thorough evaluation by a healthcare professional will help determine the most appropriate treatment and management strategies tailored to your specific situation.
