Post-operative issues after mole excision surgery?
Hello Doctor, I would like to ask – about 10 days after undergoing ligation for an internal hemorrhoid, I have developed a swollen lump about the size of a BB pellet near my anus.
It is swollen and cannot be pushed back in, and it is more painful than before the ligation.
Warm sitz baths have not provided any relief.
The doctor mentioned starting suppositories after 7 days post-surgery, but I have been unable to use them due to pain, which I suspect may be caused by the suppositories irritating the wound.
Thank you.

Dr. Ke Fangxu reply Surgery
This situation is likely due to thrombosed external hemorrhoids caused by the ligation.
It should be treated with topical medications and warm sitz baths.
If the pain and swelling persist, you may need to have the physician who performed the ligation examine you for any other issues.

After undergoing hemorrhoid banding surgery, it is not uncommon for patients to experience pain and swelling in the anal area. Your description of a swollen, painful lump the size of a BB pellet, which is more uncomfortable than before the procedure, raises several important considerations.
First, it is essential to understand that hemorrhoid banding, while generally a safe and effective procedure, can lead to post-operative complications. These may include pain, swelling, and the formation of a thrombosed hemorrhoid, which is a clot that can develop in the veins around the anus. This can result in significant discomfort and may require further treatment.
The lump you are experiencing could be a thrombosed hemorrhoid or a result of tissue swelling from the procedure. The fact that it is painful and does not reduce in size after ten days is concerning and warrants further evaluation. It is also important to note that if the lump is not reducing in size or if the pain is worsening, this could indicate that there is an issue that needs to be addressed by your healthcare provider.
Regarding the use of suppositories, if you are experiencing pain when attempting to insert them, it could indeed be due to irritation of the surgical site. The friction from the suppository against the sensitive tissue may exacerbate your discomfort. It is crucial to communicate this issue to your doctor, as they may need to adjust your treatment plan. They might suggest alternative methods for managing your pain or recommend a different type of medication that is easier to tolerate.
In addition to medication, there are several self-care strategies that may help alleviate your symptoms:
1. Warm Sitz Baths: Continue with warm sitz baths, as they can help soothe the area and promote blood flow, which may aid in healing. Ensure the water is warm but not hot, and soak for about 15-20 minutes several times a day.
2. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and reduce inflammation. However, consult your doctor before taking any medication to ensure it is appropriate for your situation.
3. Dietary Adjustments: A high-fiber diet can help prevent constipation and reduce straining during bowel movements, which can aggravate hemorrhoids. Consider incorporating more fruits, vegetables, and whole grains into your diet, and ensure you are drinking plenty of water.
4. Avoid Straining: When using the bathroom, avoid straining, as this can worsen your symptoms. If you are having difficulty with bowel movements, consider using a stool softener.
5. Follow-Up Care: Since your symptoms are persisting, it is crucial to follow up with your healthcare provider sooner than your scheduled appointment. They may need to examine the area to rule out any complications, such as an infection or a more severe issue.
In conclusion, while some discomfort and swelling can be expected after hemorrhoid banding, significant pain and the presence of a lump that does not improve should be evaluated by a healthcare professional. Open communication with your doctor about your symptoms and any difficulties you are experiencing with treatment is essential for effective management and recovery.
