Inquiring about complications related to eczema caused by a hot and humid work environment?
The working environment is hot.
I have a history of hemorrhoids and now I am experiencing anal itching, as well as itching in the perineum and groin areas.
I initially thought it was just eczema, but after visiting several clinics without improvement, I went to see Dr.
Hsieh Cheng-Chang and used topical treatments for six months.
However, I still experience occasional sharp pain in the perineum and groin, and there has been no improvement.
I feel pain when wiping, washing, or applying pressure.
I truly don’t know what to do.
I usually feel very sleepy around two or three in the morning and go straight to sleep; otherwise, the itching and pain keep me awake.
I have not found relief at clinics or hospitals, and I am feeling extremely desperate.

Dr. Chen Jiansheng reply Urology
Hello, sir: It sounds like your symptoms are quite severe.
Generally, if it is prostatitis, skin itching is less common and there may be other associated issues.
I recommend that you schedule an appointment with the urology department at a medical center for further examination.

It sounds like you are experiencing a complex set of symptoms that are affecting your quality of life significantly. Heat-related eczema, particularly in a high-temperature work environment, can lead to various complications, especially when combined with other factors such as obesity and pre-existing conditions like hemorrhoids.
Eczema, or atopic dermatitis, is a chronic skin condition that can be exacerbated by heat and humidity. In your case, the itching and discomfort in the anal and groin areas could be a manifestation of this condition, but it may also indicate other underlying issues. The fact that you have tried multiple treatments without success suggests that a more comprehensive approach may be necessary.
Firstly, it is essential to address the environmental factors contributing to your symptoms. If your workplace is consistently hot, consider ways to improve your comfort, such as wearing breathable clothing, using fans, or taking regular breaks in cooler areas. Staying hydrated is also crucial, as dehydration can exacerbate skin issues.
Regarding your symptoms of itching and pain, it is important to differentiate between eczema and other potential conditions, such as fungal infections, which are common in warm, moist areas of the body. A thorough examination by a dermatologist or a urologist may be necessary to rule out infections or other skin conditions. They may recommend specific antifungal or antibacterial treatments if they suspect an infection.
In addition to topical treatments, consider the following strategies:
1. Moisturization: Regularly applying a fragrance-free moisturizer can help maintain skin hydration and barrier function, which is crucial for managing eczema.
2. Antihistamines: Over-the-counter antihistamines may help alleviate itching, especially at night, allowing for better sleep.
3. Dietary Considerations: Sometimes, certain foods can trigger eczema flare-ups. Keeping a food diary to track your diet and symptoms may help identify any potential triggers.
4. Weight Management: Since obesity can exacerbate skin conditions, working towards a healthier weight through diet and exercise may improve your symptoms. Consulting with a nutritionist or a healthcare provider for a tailored plan could be beneficial.
5. Stress Management: Stress can worsen eczema symptoms. Techniques such as mindfulness, yoga, or other relaxation methods may help reduce stress levels.
6. Follow-Up Care: If you have not seen improvement after several months of treatment, it may be time to revisit your healthcare provider. They may consider alternative therapies, such as phototherapy or systemic medications, depending on the severity of your condition.
7. Specialist Referral: If you have not already done so, consider seeking a referral to a specialist who focuses on skin conditions or urological issues. They may have more targeted treatments available.
It is understandable to feel overwhelmed by these persistent symptoms, but it is crucial to continue seeking help and advocating for your health. Documenting your symptoms, treatment attempts, and any changes can provide valuable information to your healthcare providers, helping them to tailor a more effective treatment plan for you. Remember, you are not alone in this, and there are professionals who can help you find relief.
