Post-operative care after hemorrhoid ligation surgery?
Hello, Doctor Ke.
I underwent a rubber band ligation for internal hemorrhoids on the 30th of last month.
I read online that hemorrhoids typically necrose and fall off within seven to ten days, but it has now been 12 days since my procedure, and I can still feel a small lump at the anal opening.
The lump has been gradually decreasing in size day by day, and I haven't experienced any bleeding or pain during bowel movements.
I previously had some discharge, but it has started to decrease today.
Is this normal? Is it possible that the hemorrhoidal tissue just hasn't fallen off yet, and it might do so on its own in a few more days? I appreciate your help; I'm really worried that the ligation wasn't successful.

After hemorrhoid ligation, as long as there is no significant bleeding, there is no need to worry.
If you have done some research, you should know that it is not necessary to be concerned about whether the hemorrhoid has fallen off.
Mild discharge is a normal occurrence.
If there is no further bleeding and no pain, the ligation procedure is likely successful.

After undergoing a hemorrhoid banding procedure, it is common to have concerns about the healing process and what to expect in the days and weeks following the surgery. Based on your description, it seems that you are experiencing a typical recovery pattern, but let’s break down your concerns and provide some additional insights.
Firstly, it is important to understand that the process of healing after hemorrhoid banding can vary significantly from person to person. The banding procedure involves placing a rubber band around the base of the hemorrhoid, which restricts blood flow to the tissue. Over time, the tissue becomes necrotic (dies) and eventually falls off, usually within a week to ten days. However, in some cases, it may take a bit longer for the tissue to detach, and this can be perfectly normal.
You mentioned that it has been 12 days since your procedure, and you can still feel a small lump at the anal area. This lump is likely the banded hemorrhoid that has not yet fallen off. The fact that it is gradually getting smaller is a positive sign, indicating that the tissue is undergoing the necrosis process. The absence of bleeding and pain is also encouraging, as these symptoms can indicate complications if they were present.
Regarding the discharge you experienced, it is not uncommon to have some secretions after the procedure. As the tissue begins to die and eventually falls off, there may be some fluid discharge, which should decrease as healing progresses. The reduction in discharge you noted is a good sign that your body is healing.
To address your specific questions:
1. Is it normal for the hemorrhoid to still be present after 12 days? Yes, it can take longer than the typical 7-10 days for the tissue to fall off. As long as you are not experiencing significant pain or bleeding, this is generally not a cause for concern.
2. Will the hemorrhoid fall off on its own? Yes, it is likely that the banded hemorrhoid will eventually fall off on its own. Continue to monitor the size and any associated symptoms. If it continues to shrink and you remain symptom-free, you can expect it to detach soon.
3. What should you do if the lump does not go away? If the lump persists beyond a few more days or if you start experiencing pain, bleeding, or any other concerning symptoms, it would be wise to follow up with your healthcare provider. They can assess whether the healing is progressing as expected or if any intervention is needed.
4. What are the signs of complications? While complications are rare, you should be aware of signs such as severe pain, significant bleeding, fever, or any unusual changes in your symptoms. If you experience any of these, seek medical attention promptly.
In summary, your current experience seems to align with a normal recovery trajectory after hemorrhoid banding. Continue to monitor your symptoms, maintain good hygiene, and follow any post-operative care instructions provided by your healthcare provider. If you have any lingering concerns or if your symptoms change, do not hesitate to reach out for further evaluation. Your peace of mind is important, and your healthcare team is there to support you through your recovery.
