Discomfort in the lower left abdomen?

Hello doctor, I feel pain in my lower left abdomen when I press on it, and it hurts more than the right side.
Sometimes it feels okay, and at times I feel discomfort even without pressing.
It's not severe pain, just an indescribable discomfort.
Could this be related to my low intake of high-fiber foods?

Yi Qi, 10~19 year old female. Ask Date: 2016/08/12

Dr. Chen Qifeng reply Gastroenterology and Hepatology

Prioritize monitoring bowel movements.
If the stool is hard and in small amounts, then the left lower abdominal pain is indeed related to poor bowel habits.
However, if the pain persists, it is necessary to consult a gastroenterologist.

More Info

Left lower abdominal discomfort can be a perplexing issue, as it may arise from various underlying causes. The symptoms you describe—pain upon pressure, intermittent discomfort, and a sense of unease—can be linked to several gastrointestinal and other systemic conditions.
Firstly, the left lower abdomen houses several important structures, including parts of the colon (specifically the descending colon and sigmoid colon), the left ovary and fallopian tube in females, and the left kidney. Therefore, discomfort in this area can stem from issues related to any of these organs.

Possible Causes of Left Lower Abdominal Discomfort:
1. Gastrointestinal Issues:
- Diverticulitis: This condition occurs when diverticula (small pouches that can form in the colon) become inflamed or infected. Symptoms often include localized pain, especially in the lower left abdomen, along with changes in bowel habits.

- Irritable Bowel Syndrome (IBS): IBS can cause cramping, abdominal pain, bloating, and changes in bowel habits. Stress and dietary factors, including low fiber intake, can exacerbate symptoms.

- Constipation: Insufficient fiber intake can lead to constipation, which may cause discomfort and pain in the abdomen due to stool buildup.

2. Gynecological Issues (in females):
- Ovarian Cysts: These fluid-filled sacs can develop on the ovaries and may cause pain, especially if they rupture or become large.

- Endometriosis: This condition occurs when tissue similar to the lining of the uterus grows outside of it, often causing significant pain during menstruation or ovulation.

3. Urinary Tract Issues:
- Kidney Stones: These can cause severe pain that may radiate to the lower abdomen and groin, often accompanied by urinary symptoms.

- Urinary Tract Infections (UTIs): While more commonly associated with lower abdominal discomfort, they can also cause pain in the left lower quadrant if the left kidney is affected.

4. Musculoskeletal Issues:
- Muscle Strain: Overexertion or injury to the abdominal muscles can lead to localized pain, which may be exacerbated by movement or pressure.

Dietary Considerations:
Your mention of low fiber intake is particularly relevant. A diet low in fiber can lead to constipation, which can cause discomfort and pain in the abdomen. Fiber helps to regulate bowel movements and can prevent the buildup of stool, which may alleviate some of the discomfort you are experiencing. Increasing your intake of fruits, vegetables, whole grains, and legumes can improve your digestive health and potentially reduce symptoms.

Recommendations:
1. Dietary Changes: Gradually increase your fiber intake to promote regular bowel movements. Ensure you drink plenty of water to help fiber do its job effectively.

2. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their intensity, and any associated factors (like food intake or physical activity).

3. Consult a Healthcare Provider: Given the persistence of your symptoms, it is advisable to consult a healthcare professional. They may recommend diagnostic tests such as blood tests, imaging studies (like an ultrasound or CT scan), or a colonoscopy to rule out serious conditions.

In conclusion, while your symptoms may be related to dietary habits, they could also indicate underlying medical issues that require attention. It is essential to seek professional advice to ensure an accurate diagnosis and appropriate management.
