Loose stools?

Dear Doctor: I do not have constipation issues, but I often experience loose and unformed stools.
How can I improve this condition? Thank you.

S, 30~39 year old female. Ask Date: 2016/08/12

Dr. Li Zhonghuai reply Surgery

Hello! First, we need to determine whether the loose and unformed stools have been occurring for a long time or if this is a recent change.
Is there any presence of blood in the stool or black stools? Any change in bowel habits must first rule out the possibility of colorectal tumors, so please visit a colorectal surgery clinic for evaluation.
If the examination shows no issues, I would not recommend taking anti-diarrheal medications to firm up the stools, as this could lead to constipation, which can be more troublesome.
Take care! Changhua Hospital cares about you!

Loose stools, or diarrhea, can be a frustrating and uncomfortable issue, especially when it occurs frequently without the presence of constipation. There are several potential causes for loose stools, including dietary factors, infections, stress, and underlying medical conditions. Here are some strategies to help improve loose stools without constipation issues:
1. Dietary Adjustments:
- Bland Diet: Consider following a bland diet, often referred to as the BRAT diet (Bananas, Rice, Applesauce, Toast). These foods are low in fiber and can help firm up stools.

- Avoid Trigger Foods: Identify and avoid foods that may trigger loose stools. Common culprits include dairy products (especially if lactose intolerant), high-fat foods, spicy foods, caffeine, and artificial sweeteners like sorbitol and mannitol.

- Increase Soluble Fiber: Foods high in soluble fiber, such as oats, bananas, and psyllium, can help absorb excess water in the intestines and form more solid stools.

2. Hydration:
- It’s crucial to stay hydrated, especially if you experience loose stools frequently. Drink plenty of fluids, but avoid caffeinated and alcoholic beverages, as they can worsen dehydration. Oral rehydration solutions or electrolyte drinks can be beneficial if diarrhea is persistent.

3. Probiotics:
- Probiotics are beneficial bacteria that can help restore the natural balance of gut flora. Incorporating probiotic-rich foods like yogurt, kefir, sauerkraut, and kimchi into your diet may help improve stool consistency. Probiotic supplements can also be considered, but consult with a healthcare provider for recommendations.

4. Stress Management:
- Stress and anxiety can significantly impact gut health and lead to loose stools. Engaging in stress-reducing activities such as yoga, meditation, or deep-breathing exercises can be beneficial.

5. Medications:
- Over-the-counter medications like loperamide (Imodium) can help slow down bowel movements and reduce the frequency of loose stools. However, these should be used cautiously and not as a long-term solution. If loose stools persist, consult a healthcare provider for further evaluation.

6. Underlying Conditions:
- If loose stools are a recurring issue, it’s essential to rule out any underlying medical conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or infections. A healthcare provider may recommend tests or a referral to a gastroenterologist for further evaluation.

7. Regular Eating Schedule:
- Eating at regular intervals can help regulate bowel movements. Skipping meals or eating erratically can lead to digestive disturbances.

8. Monitor Food Intolerances:
- Keep a food diary to track what you eat and any symptoms that follow. This can help identify potential food intolerances or allergies that may be contributing to loose stools.

In summary, improving loose stools without constipation involves a combination of dietary changes, hydration, stress management, and possibly the use of probiotics or medications. If symptoms persist or worsen, it’s essential to seek medical advice to rule out any serious underlying conditions. Always consult with a healthcare professional before making significant changes to your diet or starting new treatments.
