Erectile Dysfunction: Seeking Effective Diagnosis and Treatment - Urology

Erectile Dysfunction Consultation

Doctor, I am 29 years old and I have difficulty achieving an erection.
Even when I have the time, the firmness is not normal.
This has been ongoing for several years.
I initially thought it might be due to liver function abnormalities, but after consulting with a urologist, I was told that this is not the case.
No matter what methods I try, including masturbation, I cannot achieve a normal erection, and I haven't experienced morning erections in a long time.
I have seen two urologists in the Greater Taipei area, and they have only prescribed medications like Viagra and Cialis, which have not helped at all.

I would like to know where I can get the following related tests done:
- Penile erection test using visual and pharmacological stimulation
- Nocturnal penile tumescence monitoring
- Penile blood flow assessment using Doppler ultrasound
Every time I visit a doctor, they just tell me to try the medication first, but they haven't identified the underlying cause.
I do not have hypertension, diabetes, or high cholesterol; I maintain a standard physique and exercise weekly, and my cholesterol levels and other health markers are normal.
I don't understand why those doctors prescribe medications for peripheral vascular issues and antidepressants when they have provided no help at all.

Dr. Fu Shizhang reply Urology

In general, the most common cause in young men is indeed psychogenic, meaning it is caused by psychological factors.
It is recommended to perform Doppler ultrasound or nocturnal penile tumescence monitoring to rule out organic issues.
Most medical centers offer these types of examinations.

Erectile dysfunction (ED) is a common condition that can affect men of all ages, including younger individuals like yourself. At 29, experiencing difficulties with erections can be concerning, especially when it persists for several years. It's important to understand that ED can have multiple underlying causes, including psychological, hormonal, neurological, and vascular factors.

From your description, it seems you've already consulted with urologists who prescribed medications like Viagra (sildenafil) and Cialis (tadalafil), but these have not provided the relief you were hoping for. It's essential to recognize that while these medications can be effective for many men, they do not address the root cause of the problem. Since you mentioned that you have normal cholesterol levels, maintain a healthy weight, and exercise regularly, it suggests that lifestyle factors may not be the primary issue.

Given your situation, it would be beneficial to explore further diagnostic evaluations to identify any underlying causes of your ED. Here are some tests and evaluations that you might consider discussing with your healthcare provider:
1. Hormonal Testing: A blood test to check testosterone levels can be crucial, as low testosterone can contribute to erectile dysfunction. Other hormone levels, such as thyroid hormones and prolactin, may also be evaluated.

2. Nocturnal Penile Tumescence (NPT) Test: This test measures erections that occur during sleep. Normal results may indicate that the ED is more likely to be psychological rather than physical.

3. Penile Doppler Ultrasound: This test assesses blood flow to the penis. It can help determine if there are vascular issues contributing to your erectile dysfunction.

4. Psychological Evaluation: Since psychological factors can play a significant role in ED, a mental health evaluation may be beneficial. Stress, anxiety, and depression can all impact sexual function.

5. Urinalysis and Blood Tests: These can help rule out diabetes or other metabolic conditions that could affect erectile function.

6. Medication Review: Some medications, including those for high blood pressure or depression, can contribute to ED. A thorough review of your current medications with your doctor may reveal potential side effects that could be impacting your sexual health.

7. Lifestyle Factors: While you mentioned that you have a healthy lifestyle, it may be worth discussing any potential stressors in your life, including work-related stress or relationship issues, which can also affect sexual performance.

If you feel that your current healthcare providers are not adequately addressing your concerns, it may be worthwhile to seek a second opinion or consult a specialist in sexual medicine. Look for clinics that focus specifically on sexual health, as they may offer more comprehensive evaluations and treatment options tailored to your needs.

In terms of where to find these tests, many urology clinics and sexual health centers in larger metropolitan areas, such as Taipei, should offer these services. You can also inquire about specialized sexual health clinics that may provide a more focused approach to diagnosing and treating erectile dysfunction.

In conclusion, while medications can be a part of the treatment plan for erectile dysfunction, identifying and addressing the underlying causes is crucial for effective management. Don't hesitate to advocate for yourself and seek out the tests and evaluations that can help you find the answers you need.
