Doctor, may I ask about the skin on my testicles?

Hello, doctor.
I would like to ask about the white bumps on the skin of my testicles.
There is one large and one small bump that have been present for about six or seven years.
Sometimes they disappear for several months, but they have reappeared recently.
I would like to know what these are.
If I want to remove them for cosmetic reasons, would electrosurgery or cryotherapy be a better option? Thank you for your response.

A-Ming, 30~39 year old female. Ask Date: 2016/08/17

Dr. Sun Hongshun reply Urology

Hello, it looks like miliaria, but it really requires a physical examination to confirm.
It may be possible to treat it with needle aspiration, excision, cryotherapy, or laser treatment.
It is recommended to consult a dermatologist for further management.

The white bumps you are observing on the skin of your testicles could be sebaceous cysts, pearly penile papules, or other benign skin lesions. These conditions are generally harmless, but understanding their nature and treatment options is essential for your peace of mind and health.

Causes of White Bumps on Testicular Skin:
1. Sebaceous Cysts: These are small, non-cancerous lumps beneath the skin, often filled with a fatty substance called sebum. They can appear anywhere on the body, including the genital area, and may fluctuate in size or even disappear over time.

2. Pearly Penile Papules: These are small, dome-shaped bumps that typically appear around the corona of the glans penis. They are a normal anatomical variant and are not associated with any disease or infection.

3. Folliculitis: This is an inflammation of the hair follicles, which can lead to small white or red bumps. It can be caused by bacterial infections, irritation, or ingrown hairs.

4. Milia: These are tiny white cysts that can occur when keratin becomes trapped beneath the surface of the skin. They are common in various areas of the body, including the genital region.

5. Genital Warts: Caused by the human papillomavirus (HPV), these can appear as small, flesh-colored or white bumps. They may require medical evaluation and treatment.

Treatment Options:
If the bumps are causing you concern regarding aesthetics or discomfort, there are several treatment options available:
1. Electrosurgery: This method uses electrical currents to remove the bumps. It is effective for various types of skin lesions and can provide immediate results. However, it may leave some scarring.

2. Cryotherapy: This treatment involves freezing the lesions with liquid nitrogen. It is often used for warts and some types of cysts. Cryotherapy can be effective but may require multiple sessions for complete removal.

3. Laser Therapy: This is another option for removing unwanted skin lesions. Laser treatment can be precise and minimize damage to surrounding tissues, reducing the risk of scarring.

4. Topical Treatments: Depending on the diagnosis, topical treatments may be prescribed. For example, if the bumps are due to folliculitis, antibiotics or anti-inflammatory creams may be recommended.

5. Observation: If the bumps are asymptomatic and not causing any issues, your healthcare provider may recommend simply monitoring them, as many benign conditions do not require treatment.

Self-Care and Considerations:
- Avoid squeezing or picking at the bumps, as this can lead to infection or scarring.

- Maintain good hygiene in the genital area to prevent irritation or infection.

- If you notice any changes in the size, color, or texture of the bumps, or if they become painful, seek medical advice.

In conclusion, while the white bumps on your testicular skin may be benign, it is always best to consult with a healthcare professional for an accurate diagnosis and appropriate treatment options tailored to your specific situation. If you are considering treatment for cosmetic reasons, discuss the pros and cons of each method with your doctor to make an informed decision.
