Bowel movement issues?

Hello, Director Hsu: Every time I have a bowel movement, I experience constipation and have to strain to pass stool, which comes out in hard, pellet-like pieces.
Afterward, my anus often cracks, and sometimes the stool is so large that the toilet water turns red, and there is fresh blood from my anus.
Most importantly, after I finish, there are fleshy bumps around my anus that look like blisters; they are not painful or itchy and disappear within half an hour to an hour.
How can I resolve this issue to prevent those bumps from appearing?

Dr. Xu Kaixi reply Surgery

Based on your description, this situation is more indicative of hemorrhoids (internal or external).
It is recommended to visit a surgical outpatient clinic for further examination and diagnosis.

Constipation and anal issues can be quite distressing and uncomfortable, and it’s important to address them effectively to prevent further complications. Based on your description, it seems you are experiencing significant constipation, which is leading to painful bowel movements and the appearance of what you describe as "bubbles" or swollen tissue around the anus. This could potentially be related to hemorrhoids or anal fissures, both of which are common conditions associated with constipation.

Understanding Your Symptoms
1. Constipation: This is characterized by infrequent bowel movements or difficulty passing stools. When stools are hard and dry, they can cause pain and discomfort during bowel movements. The straining you describe can lead to anal fissures, which are small tears in the lining of the anus, causing bleeding and pain.

2. Hemorrhoids: These are swollen veins in the lower rectum and anus that can become inflamed due to straining during bowel movements. They can cause discomfort, itching, and bleeding, especially if the stool is hard and requires significant effort to pass.

3. Anal Skin Tags or Swelling: The "bubbles" you mention could be skin tags or swollen tissue that sometimes appear after straining. They are usually harmless but can be bothersome.

Solutions for Relief
1. Dietary Changes: Increasing your fiber intake is crucial. Aim for a diet rich in fruits, vegetables, whole grains, and legumes. Fiber helps to soften the stool and promotes regular bowel movements. Additionally, ensure you are drinking plenty of water—at least 8-10 glasses a day—to help keep your stool soft.

2. Regular Exercise: Physical activity can stimulate bowel function. Aim for at least 30 minutes of moderate exercise most days of the week.

3. Establish a Routine: Try to set aside time each day for bowel movements, preferably after meals when the digestive system is most active. This can help train your body to have regular bowel movements.

4. Over-the-Counter Remedies: You might consider using a stool softener or a mild laxative as needed. Products containing docusate sodium (stool softener) or polyethylene glycol (osmotic laxative) can be effective. However, these should not be used as a long-term solution without consulting a healthcare provider.

5. Warm Baths: Soaking in a warm bath can help relieve discomfort and promote relaxation of the anal area, which may help with bowel movements.

6. Avoid Straining: When you feel the urge to go, try not to delay it. Straining can exacerbate fissures and hemorrhoids. If you find it difficult to pass stool, consider using a footstool to elevate your feet while sitting on the toilet, which can help align the rectum for easier passage.

7. Consult a Healthcare Provider: Given your symptoms, especially the presence of blood in your stool and the appearance of swelling, it is essential to consult a healthcare provider. They may recommend further evaluation, such as a physical examination or possibly a colonoscopy, to rule out any serious conditions.

8. Topical Treatments: If you are experiencing pain or discomfort from hemorrhoids or fissures, topical treatments such as hydrocortisone cream or witch hazel pads can provide relief.

Conclusion
It’s important to address constipation and anal issues promptly to prevent complications such as chronic pain, bleeding, or more severe conditions. By making dietary changes, establishing a regular routine, and consulting with a healthcare provider, you can find relief from your symptoms and improve your overall digestive health. If your symptoms persist or worsen, do not hesitate to seek medical attention, as they can provide tailored advice and treatment options based on your specific situation.
