Cardiac issues?

Hello doctor, recently I have been very aware of my heartbeat, especially at night when I can feel a pronounced thumping sensation, around 80 beats per minute.
My heart sometimes feels sore and prickly, and there are times when I also experience sharp pains in my chest, but these pains usually last less than five minutes.
This sensation is more pronounced at night, becoming increasingly noticeable as it gets later.
However, this does not happen every day.
I also feel a pressure in my chest, as if something is compressing it, along with a tight and suffocating sensation.
I often experience chest tightness and shortness of breath, needing to take deep breaths to feel somewhat better.
Sometimes, even without exertion, I feel like my breathing is not smooth and I am short of breath.
Occasionally, my heart feels fatigued.
Should I urgently check my heart? Do these symptoms affect the blood circulation in my left arm? Because when my left arm is in a normal position without any pressure, I often feel a tingling and numb sensation extending from the inner side of my left arm to my palm, similar to the feeling of poor blood circulation.
Additionally, my left palm appears noticeably less pink than my right palm.
I would like to know if this is related to heart issues or purely a matter of blood circulation.

Dr. He Dongjin reply Cardiology

Hello, the palpitations are likely symptoms caused by autonomic nervous system dysregulation.
It is advisable to continue monitoring the condition.
If discomfort persists, it is recommended to visit an outpatient clinic for further examination.

When it comes to heart symptoms, understanding when to seek medical attention is crucial for your health and well-being. Based on the symptoms you've described, there are several important factors to consider.

Firstly, the sensation of your heart beating strongly, especially at night, can be a sign of palpitations. While palpitations can be benign and often related to anxiety, caffeine intake, or lack of sleep, they can also indicate underlying heart issues, particularly if accompanied by other symptoms such as chest pain, shortness of breath, or dizziness. Your reported symptoms of sharp, stabbing chest pain that lasts for a few minutes, along with a feeling of pressure in the chest, are concerning and warrant further evaluation.

Chest pain, especially when it is associated with other symptoms like shortness of breath, can be indicative of serious conditions such as angina or even a heart attack. Angina is characterized by chest pain due to reduced blood flow to the heart muscle, often triggered by physical exertion or stress. If your symptoms occur during rest or are accompanied by sweating, nausea, or pain radiating to the arm, jaw, or back, it is imperative to seek immediate medical attention.

The feeling of tightness or pressure in your chest, along with the need to take deep breaths, could also suggest anxiety or panic attacks, which can mimic heart-related symptoms. However, distinguishing between anxiety and a cardiac issue can be challenging without professional evaluation.
Regarding your concern about the left arm's blood circulation, the symptoms of numbness and tingling in the left arm, especially when combined with chest pain, can be a red flag. While these sensations can arise from various causes, including nerve compression or poor circulation, they can also be associated with cardiac issues. The left arm is often involved in heart-related symptoms, particularly in men, but women can also experience these signs.
Given your symptoms, it is advisable to consult a healthcare professional for a thorough evaluation. A cardiologist can perform tests such as an electrocardiogram (ECG), echocardiogram, or stress test to assess your heart's function and rule out any serious conditions. Additionally, blood tests may be conducted to check for markers of heart damage or other underlying issues.

In summary, while some of your symptoms may be benign, the combination of chest pain, palpitations, and left arm discomfort should not be ignored. It is always better to err on the side of caution when it comes to heart health. If you experience worsening symptoms, such as increased frequency or intensity of chest pain, shortness of breath, or any new symptoms, seek emergency medical care immediately. Your health is paramount, and timely intervention can make a significant difference in outcomes related to heart conditions.
