Severe constipation?

Hello doctor, I have been experiencing severe constipation lately, along with increased bloating and flatulence.
My abdomen feels a bit painful (frequently), and although I have the urge to have a bowel movement, I only pass gas when I go to the bathroom.
I have been drinking 2000cc of water and eating vegetables, but I exercise very little.
What could be the situation?

Hyf, 20~29 year old female. Ask Date: 2016/08/28

Dr. Yang Zhichao reply Gastroenterology and Hepatology

Hello Ms.
Hyf, based on the situation you provided, the constipation may be due to poor gastrointestinal motility.
You should have a doctor evaluate your condition, and it's important to improve your lifestyle by incorporating more exercise.
Thank you for your inquiry.

More Info

Severe constipation can be a distressing condition, and it often comes with a range of uncomfortable symptoms, including bloating, increased flatulence, and abdominal pain. Based on your description, it seems you are experiencing significant discomfort, and it's important to understand the potential causes and solutions for your situation.

Causes of Severe Constipation
1. Dietary Factors: While you mentioned consuming vegetables and drinking about 2000cc of water, it's crucial to ensure that your diet is rich in fiber. Fiber helps to bulk up stool and promotes regular bowel movements. If your diet lacks adequate fiber, it can lead to constipation. Foods high in fiber include fruits, whole grains, legumes, and nuts.

2. Lack of Physical Activity: You noted that you are not exercising much. Physical activity is essential for stimulating intestinal function. Regular exercise can help move food through the digestive system and prevent constipation.

3. Dehydration: Although you are drinking a significant amount of water, it’s important to ensure that you are adequately hydrated throughout the day. Dehydration can lead to harder stools, making them more difficult to pass.

4. Medications: Certain medications can contribute to constipation. If you are taking any medications, especially painkillers, antacids containing aluminum, or some antidepressants, these could be affecting your bowel movements.

5. Medical Conditions: Various medical conditions can lead to constipation, including hypothyroidism, diabetes, irritable bowel syndrome (IBS), and neurological disorders. If constipation persists, it may be worth discussing these possibilities with a healthcare provider.

6. Stress and Anxiety: Psychological factors can also play a role in digestive health. Stress and anxiety can affect gut motility and lead to constipation.

Solutions for Severe Constipation
1. Increase Fiber Intake: Aim for a daily intake of 25-30 grams of fiber. Gradually increase your fiber intake to avoid gas and bloating. Consider incorporating foods like oats, beans, lentils, fruits (like apples and pears), and vegetables (like broccoli and carrots).

2. Stay Hydrated: Continue to drink plenty of water, but also consider other fluids like herbal teas or broths. Sometimes, warm liquids can help stimulate bowel movements.

3. Regular Exercise: Incorporate physical activity into your daily routine. Even simple activities like walking, stretching, or yoga can help stimulate your digestive system.

4. Establish a Routine: Try to set a regular time each day to use the bathroom, especially after meals. This can help train your body to have regular bowel movements.

5. Over-the-Counter Solutions: If dietary changes and lifestyle modifications do not help, consider over-the-counter laxatives or stool softeners. However, these should be used sparingly and not as a long-term solution.

6. Consult a Healthcare Provider: If your symptoms persist or worsen, it’s essential to consult a healthcare provider. They can conduct a thorough evaluation, including a physical examination and possibly further testing, to determine the underlying cause of your constipation.

7. Mindfulness and Stress Management: If stress is a contributing factor, consider practices like mindfulness, meditation, or yoga to help manage anxiety and improve gut health.

Conclusion
Severe constipation can significantly impact your quality of life, and addressing it requires a multifaceted approach. By focusing on dietary changes, increasing physical activity, and consulting with a healthcare provider if necessary, you can work towards alleviating your symptoms. Remember, it’s essential to listen to your body and seek professional guidance if your condition does not improve.
