What to do about severe constipation?
Doctor, my constipation has become more severe recently.
It's only once a week! (I can only push out a little bit!) I've also been taking Wakamoto, but it hasn't noticeably improved my constipation.
I've been under a lot of stress lately, and my diet and sleep have been irregular! How can I improve my constipation problem? Thank you, doctor.

Dr. Lin Minghui reply Gastroenterology and Hepatology
Hello: For severe constipation, it is recommended to adjust your diet by increasing fluid intake, consuming more fiber, and possibly supplementing with probiotics.
It's also important to relax and develop good bowel habits, along with engaging in moderate exercise.
If constipation is severe, medication treatment may be necessary.
It is advisable to follow up at the outpatient clinic.
Wishing you good health.
Sincerely, Dr.

More Info
Severe constipation can be a frustrating and uncomfortable condition, especially when it significantly impacts your daily life. It’s important to understand that constipation is defined as having fewer than three bowel movements per week, and it can be caused by a variety of factors including diet, lifestyle, medications, and underlying health conditions. Given your situation, where you are experiencing bowel movements only once a week, it is essential to take a comprehensive approach to address this issue.
Dietary Changes
1. Increase Fiber Intake: A diet rich in fiber can help improve bowel regularity. Aim for at least 25-30 grams of fiber per day. Foods high in fiber include fruits (like apples, pears, and berries), vegetables (such as broccoli, carrots, and leafy greens), whole grains (like oats, brown rice, and whole wheat bread), and legumes (beans, lentils, and chickpeas).
2. Stay Hydrated: Drinking plenty of water is crucial for preventing constipation. Aim for at least 8-10 cups of water daily. Adequate hydration helps soften the stool, making it easier to pass.
3. Limit Processed Foods: Foods that are high in fat and low in fiber, such as fast food, chips, and pastries, can exacerbate constipation. Try to limit these foods and focus on whole, unprocessed options.
Lifestyle Modifications
1. Regular Physical Activity: Engaging in regular exercise can stimulate bowel movements. Aim for at least 30 minutes of moderate exercise most days of the week. Activities like walking, jogging, cycling, or yoga can be beneficial.
2. Establish a Routine: Try to set a regular time each day for bowel movements, especially after meals. This can help train your body to have more regular bowel habits.
3. Manage Stress: Since you mentioned that you are experiencing increased stress, it’s important to find ways to manage it. Stress can negatively impact digestion and bowel function. Consider relaxation techniques such as deep breathing, meditation, or gentle yoga.
Medical Considerations
1. Review Medications: Some medications can contribute to constipation. If you are taking any medications, including over-the-counter supplements like Wakamoto, consult with your healthcare provider to see if they could be affecting your bowel movements.
2. Consider Laxatives: If dietary and lifestyle changes do not improve your symptoms, you may want to discuss the use of over-the-counter laxatives with your doctor. Options include fiber supplements, osmotic laxatives (like polyethylene glycol), or stimulant laxatives (like bisacodyl). However, these should be used under medical supervision, especially if you have other health conditions.
3. Consult a Specialist: If your constipation persists despite making these changes, it may be beneficial to consult a gastroenterologist. They can perform further evaluations to rule out any underlying conditions such as irritable bowel syndrome (IBS), hypothyroidism, or other gastrointestinal disorders.
Conclusion
In summary, addressing severe constipation requires a multifaceted approach that includes dietary changes, lifestyle modifications, and possibly medical interventions. By increasing your fiber intake, staying hydrated, exercising regularly, and managing stress, you can significantly improve your bowel health. If these strategies do not yield results, do not hesitate to seek professional medical advice to explore further options. Remember, you are not alone in this, and effective solutions are available to help you regain your comfort and well-being.
