Postoperative discharge?
Hello, doctor.
The physician said that my abscess is quite superficial and that a fistula has formed during the examination.
Therefore, at the beginning of the month, I underwent a procedure for anal fistula drainage and incision.
The surrounding area has been sutured with 2-3 stitches, and it has been 26 days post-surgery.
Most of the wound area and the sutured parts have healed and developed skin, but there is still a deeper hole near the anus that looks quite deep.
By the second week, I was already experiencing less pain, and by the third week, I stopped taking any pain medication.
However, I still take sitz baths every morning and evening, and after using the bathroom, I apply Neomycin ointment.
But now, every time I dry off after the sitz bath, there is still some greenish discharge.
Is this normal? I'm really worried that it hasn't healed properly.
Also, if the surface of the skin looks healed, do I still need to continue with the sitz baths? Thank you.

Dr. Ke Fangxu reply Surgery
After anal fistula surgery, the wound typically takes about a month to heal.
It is normal to have some discharge from the wound during this time.
If the wound becomes infected, you may experience redness, swelling, heat, and pain.
If these symptoms are not present, there is no need for concern.
Sitz baths are generally recommended to continue until there is no more discharge.

After undergoing surgery for an anal fistula, it is completely understandable to have concerns about the healing process and the signs of normal versus abnormal recovery. Based on your description, it seems that you are experiencing a typical postoperative scenario, but let’s break down the details to provide clarity.
Firstly, the fact that most of your wound has healed and that you are no longer experiencing significant pain is a positive sign. Healing after surgery can vary significantly from person to person, and it sounds like you are progressing well. The presence of a deep area near the anus that has not fully healed is not uncommon, especially in the case of anal fistula surgeries, where the tissue can take time to regenerate fully.
Regarding the greenish discharge you are noticing, it is essential to consider a few factors. Discharge can be a normal part of the healing process, especially if it is minimal and not accompanied by other symptoms such as increased pain, swelling, or fever. The green color could indicate the presence of old blood or a normal inflammatory response as your body works to heal the tissue. However, if the discharge is excessive, has a foul odor, or is accompanied by other concerning symptoms, it may indicate an infection, and you should reach out to your healthcare provider for further evaluation.
As for the practice of sitz baths, they are often recommended post-surgery for several reasons. Sitz baths can help keep the area clean, promote blood flow to the region, and aid in the healing process. Even if the surface of the skin appears to be healing, the underlying tissues may still be in the process of recovery. Continuing with sitz baths can help maintain hygiene and comfort, especially if you are experiencing any discomfort or if the area is still discharging.
In summary, while it sounds like you are on the right track with your recovery, it is crucial to monitor the discharge and any other symptoms closely. If you notice any changes that concern you, such as increased pain, a change in the color or odor of the discharge, or if the depth of the wound does not seem to improve, do not hesitate to contact your surgeon or healthcare provider. They can provide specific guidance based on a physical examination and your individual healing process.
In conclusion, postoperative care is vital, and your proactive approach to monitoring your recovery is commendable. Keep up with your sitz baths, maintain good hygiene, and stay in touch with your healthcare provider to ensure a smooth recovery. If you have any further questions or concerns, don’t hesitate to reach out for professional advice.
