Medication-induced abortion, breast tenderness, and urine testing issues?

Doctor, I took the first dose of medication for a medical abortion on August 19, and I experienced bleeding the next day.
I took the second dose of medication on August 22, and there was also bleeding.
During my follow-up appointment on August 25, the doctor examined me and said that most of the tissue had been expelled, but it wasn't completely clear.
That day, I was prescribed medication for three days and was instructed to return for a follow-up in a week.

At my follow-up on September 3, the doctor noted that a small amount of tissue remained, but it was not significant enough to cause any issues and said it was fine.
A urine test was conducted, and the doctor confirmed that I was no longer pregnant, although I did not see the test line myself.
I was given another three days of medication and advised that if I did not experience any bleeding, I did not need to return until my next menstrual period.
After finishing the medication, I did not have any bleeding, but I still felt persistent breast tenderness.
Concerned that I might still be pregnant, I took a home pregnancy test.
Initially, I did not see a second line, but later that evening, I noticed a faint second line.
Doctor, does this indicate that the medication was not fully effective? Thank you.

Hao, 30~39 year old female. Ask Date: 2016/09/09

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello Ms.
Hao: The faint second line on your urine test result is completely normal and does not indicate that you have not cleared the pregnancy hormones from your body.
Typically, it takes about three to four weeks for the residual pregnancy hormones to be metabolized completely.
Therefore, it is common to see a faint second line on the urine test during this period.
This does not mean that you are pregnant again or that there has been any incomplete miscarriage.

After undergoing a medical abortion, it is not uncommon for women to experience various physical symptoms, including breast tenderness. This can be particularly concerning, especially if you are unsure about whether the abortion was complete or if you might still be pregnant.
In your case, you underwent a medical abortion on August 19 and 22, with follow-up appointments indicating that most of the pregnancy tissue had been expelled, but some remnants remained. The absence of bleeding after taking the medication can be a normal response, as some women may not experience heavy bleeding after a medical abortion, especially if the process was mostly successful. However, breast tenderness can persist for a while due to hormonal changes in your body following the abortion.

Breast tenderness is often linked to hormonal fluctuations, particularly changes in estrogen and progesterone levels. After a pregnancy ends, your body needs time to adjust back to its non-pregnant state, which can lead to symptoms such as breast tenderness, mood swings, and changes in menstrual cycles. It is also worth noting that the presence of a faint line on a pregnancy test can indicate residual hCG (human chorionic gonadotropin) hormone in your system, which can linger for some time after a pregnancy ends, especially if there is any remaining tissue in the uterus.

If your healthcare provider has assured you that the remaining tissue is not significant enough to cause concern, it is likely that your body is simply adjusting to the hormonal changes. However, if you continue to experience breast tenderness or have any other symptoms that cause you concern, it is important to follow up with your healthcare provider. They may recommend an ultrasound to ensure that there are no complications or retained products of conception that could be causing your symptoms.

In summary, breast tenderness after a medical abortion can be a normal part of the recovery process due to hormonal changes. However, if you have any doubts about whether the abortion was complete or if you are experiencing unusual symptoms, it is crucial to consult with your healthcare provider for further evaluation and peace of mind. They can provide you with the best guidance based on your specific situation and medical history.
