Postoperative condition of the testicles after varicocele surgery?
Hello Doctor, it has been over twenty days since my bilateral varicocele microsurgery, and I can still feel blood vessels on the left testicle, which is more pronounced than on the right.
The left testicle is significantly larger, almost twice the size of the right one.
Pressing on the blood vessels causes severe pain, and I also experience a dull ache after standing for a long time.
Is this a normal post-operative phenomenon? Thank you for your response.

Hello, since you have undergone surgery on both sides, the recovery should ideally be similar for both.
If you are experiencing abnormal swelling and pain on one side as you described, please return to your original surgeon's office for evaluation and further management as soon as possible.
Wishing you good health.

After undergoing bilateral varicocele surgery, it is not uncommon for patients to experience various postoperative symptoms, including swelling and discomfort in the testicular area. The presence of palpable blood vessels, particularly if they are more pronounced on one side, can be concerning for patients. However, it is essential to understand that some degree of swelling and vascular prominence can be a normal part of the healing process following varicocele repair.
In your case, the left testicle appears to be significantly larger than the right, which could indicate several possibilities. First, it is important to note that post-surgical edema (swelling) is a common occurrence. The body responds to surgical trauma with inflammation, which can lead to increased fluid accumulation in the tissues surrounding the surgical site. This swelling may take several weeks to resolve, and during this time, the affected testicle may feel larger and more sensitive.
The pain you are experiencing when pressing on the blood vessels and the discomfort associated with standing for extended periods may also be attributed to the healing process. Nerve endings in the area may still be sensitive after surgery, and as the body heals, it can take time for normal sensation to return. However, if the pain is severe or worsening, it is crucial to communicate this with your healthcare provider.
Another consideration is the possibility of a recurrence of varicocele. While surgical intervention aims to correct the abnormal veins, there is a chance that some veins may remain or that new ones may develop over time. If the blood vessels you are feeling are indeed varicose veins, this could explain the persistent symptoms. A follow-up ultrasound may be warranted to assess the vascular status of the testicles and determine if any additional treatment is necessary.
It is also important to monitor for any signs of complications, such as infection or hematoma (a localized collection of blood outside of blood vessels). Symptoms to watch for include increased redness, warmth, or discharge from the surgical site, as well as fever or significant changes in pain levels. If you experience any of these symptoms, it is essential to seek medical attention promptly.
In terms of recovery, it is generally advised to avoid heavy lifting and strenuous activities for several weeks post-surgery to allow for optimal healing. Gradually increasing activity levels while listening to your body can help manage discomfort. If standing for long periods is causing significant discomfort, consider taking breaks or using supportive garments as recommended by your surgeon.
In conclusion, while some postoperative symptoms such as swelling and discomfort can be normal, it is essential to maintain open communication with your healthcare provider. Regular follow-up appointments are crucial for monitoring your recovery and addressing any concerns that may arise. If you have any doubts about your condition or if symptoms persist or worsen, do not hesitate to reach out for further evaluation and guidance. Your health and comfort are paramount, and your healthcare team is there to support you through your recovery process.
