On December 24, bilateral varicocele repair and right testicular fixation surgery were performed?
Hello, Doctor.
About two weeks ago, I underwent bilateral varicocele ligation and right testicular fixation surgery.
I have only one incision located below the scrotum (between the two testicles).
I would like to ask what type of varicocele ligation surgery this is considered? Is this surgical method associated with higher risks? Since the surgery, both testicles have been hard and swollen (especially the right one), and they feel oddly shaped upon touch but are not painful.
The follow-up physician diagnosed it as orchitis and prescribed medication.
I have been taking the medication for nearly a week, but the aforementioned condition has not improved.
Is there a risk of testicular necrosis or atrophy? Can an ultrasound confirm the current condition of the testicles? Thank you, Doctor!

Hello: You should discuss the surgical method with your physician.
It is common to experience swelling after surgery; however, if there is redness or fever in the skin, you should be alert.
Thank you for your inquiry.

More Info
After undergoing bilateral varicocele surgery and right testicular fixation, it's understandable to have concerns regarding potential risks and symptoms associated with the procedure. Varicocele surgery, often performed to alleviate symptoms related to enlarged veins in the scrotum, can be done through various techniques, including open surgery, laparoscopic surgery, or microsurgical techniques. The specific approach used can influence the risks and recovery process.
Types of Varicocele Surgery
The surgery you underwent likely involved ligation of the affected veins to redirect blood flow. If the incision is located below the scrotum, it may suggest an open surgical approach. This method is generally effective but can carry risks such as infection, bleeding, and damage to surrounding structures, including the testicular artery or vas deferens.
Post-Operative Symptoms
Post-operative symptoms can vary widely among individuals. The swelling and hardness of the testicles you are experiencing could be attributed to several factors, including:
1. Post-Surgical Edema: Swelling is common after surgery and may take time to resolve. This is often due to fluid accumulation and inflammation in the surgical area.
2. Testicular Inflammation: Your doctor mentioned testicular inflammation, which can occur post-surgery. This condition may cause discomfort and changes in the shape of the testicles but is typically manageable with medication.
3. Potential Complications: While the risk of testicular atrophy or necrosis is low, it is not entirely absent. Factors such as compromised blood supply during surgery or post-operative complications can contribute to these risks.
Monitoring and Diagnosis
Ultrasound is a valuable tool in assessing the condition of the testicles post-surgery. It can help identify any abnormalities, such as fluid collections, blood flow issues, or signs of necrosis. If you are experiencing persistent symptoms or if the swelling worsens, it is crucial to follow up with your healthcare provider for further evaluation.
Risks of Testicular Atrophy or Necrosis
The risk of testicular atrophy or necrosis post-surgery is generally low, especially if the surgery was performed correctly and there were no intraoperative complications. However, if there are concerns about blood flow or if the testicle appears significantly altered in shape or size, immediate medical evaluation is warranted.
Conclusion
In summary, while some swelling and changes in the testicular shape can be expected after varicocele surgery, persistent symptoms warrant further investigation. An ultrasound can provide reassurance and help rule out serious complications. Always communicate openly with your healthcare provider about your symptoms and concerns, as they can guide you on the best course of action for your recovery. If necessary, they may refer you to a specialist for further evaluation and management.
