Varicocele
Hello doctor, I have a question.
I just had a varicocelectomy performed laparoscopically two weeks ago.
The incision on my abdomen has not been painful for several days after discharge, but now I feel a slight pain in my abdomen towards the end of urination.
Additionally, the veins in my testicles still appear to be enlarged, and I sometimes feel discomfort, including a warm sensation, similar to before the surgery.
Is this considered a recurrence? Should I expect improvement in several months, or should I avoid erections and sexual activity? I am quite worried and would appreciate your advice on what I should pay attention to.
Thank you.

Dr. Xu Weikai reply Urology
KK: Hello, after general surgery, it usually takes about 6 months for the symptoms of varicocele to show significant improvement.
There are no special restrictions on daily activities; you can continue your usual routine as before the surgery.
This is a response from Dr.

After undergoing laparoscopic varicocele surgery, it's common to have concerns about post-operative symptoms and recovery. Based on your description, it seems you're experiencing some discomfort and changes in urination and testicular sensation, which can be concerning. Let's address your questions and provide some insights into the recovery process after this type of surgery.
1. Post-Operative Pain and Discomfort: It's normal to experience some pain or discomfort in the abdominal area and around the surgical site for a few weeks following laparoscopic surgery. The fact that your pain has decreased significantly since the surgery is a positive sign. However, experiencing pain during urination, especially towards the end, can be indicative of several factors, including irritation from the surgery or residual swelling in the area. This discomfort should gradually improve as your body heals.
2. Testicular Sensation and Blood Vessel Appearance: The sensation of warmth or discomfort in the testicular area, along with the appearance of the blood vessels remaining prominent, can be concerning. It's important to note that immediate post-operative changes can occur, and it may take time for the blood vessels to return to their normal size and for any swelling to subside. However, if the vessels remain enlarged or if you experience significant discomfort, it could warrant further investigation to rule out complications such as recurrence of varicocele or other issues.
3. Recovery Timeline: Recovery from laparoscopic varicocele surgery can vary from person to person. Generally, it may take several weeks to a few months for complete healing and for symptoms to stabilize. It's essential to give your body time to recover fully. If you continue to experience discomfort or if your symptoms worsen, it would be advisable to follow up with your surgeon or urologist for a thorough evaluation.
4. Activity Restrictions: During the initial recovery phase, it's typically recommended to avoid strenuous activities, heavy lifting, and sexual activity for a period of time, usually around 2 to 4 weeks, depending on your surgeon's advice. Engaging in these activities too soon can increase the risk of complications or prolong your recovery. It's best to follow your surgeon's specific recommendations regarding activity restrictions.
5. When to Seek Medical Attention: If you notice any of the following, you should contact your healthcare provider:
- Increased pain or swelling in the surgical area.
- Fever or chills, which could indicate an infection.
- Changes in urination, such as blood in the urine or severe pain.
- Persistent discomfort in the testicular area that does not improve.
In summary, while some discomfort and changes in sensation are common after laparoscopic varicocele surgery, it's crucial to monitor your symptoms and communicate with your healthcare provider about any concerns. They can provide personalized advice based on your specific situation and ensure that your recovery is progressing as expected. Remember, healing takes time, and it's important to be patient with your body as it recovers from surgery.
