Small ulcers on the foreskin?

On September 19, two circular small ulcers were discovered on the foreskin, accompanied by pain.
There has been discomfort in the left inguinal region for several days.
I have been applying iodine and taking anti-inflammatory medication; the ulcers are no longer painful, but the foreskin around the ulcers has become more red and swollen since the day after the ulcers appeared.
Starting from September 20, there has been a burning pain at the urethral opening during urination.
On September 23, a red rash appeared on the back of both shoulders, but it is not itchy.
What kind of condition could this be?

More Info

Understanding painful ulcers on the foreskin can be a complex issue, as several potential causes could lead to the symptoms you are experiencing. Based on your description, it seems you have developed painful circular ulcers on the foreskin, accompanied by discomfort in the left inguinal region, burning pain during urination, and a subsequent rash on your shoulders. Let’s explore some possible causes and treatments for your condition.

Possible Causes
1. Infections:
- Herpes Simplex Virus (HSV): One of the most common causes of painful ulcers in the genital area is genital herpes. The initial outbreak can present with painful sores, and it may also be accompanied by systemic symptoms such as fever and swollen lymph nodes.

- Bacterial Infections: Conditions like balanitis (inflammation of the glans) can occur due to bacterial infections, leading to ulcers and redness.

- Fungal Infections: Yeast infections can also cause irritation and ulceration, particularly in warm, moist areas.

2. Sexually Transmitted Infections (STIs):
- Conditions such as syphilis or chancroid can present with ulcers on the genitalia. If you have had unprotected sexual contact, it is essential to consider these possibilities.

3. Inflammatory Conditions:
- Lichen Planus: This is an inflammatory condition that can affect the genital area, leading to painful lesions.

- Psoriasis: While typically associated with scaly patches, psoriasis can also affect the genital area and may present with red, inflamed skin.

4. Allergic Reactions or Irritation:
- Sometimes, contact dermatitis due to soaps, lotions, or other irritants can cause redness and ulceration.

5. Systemic Conditions:
- Conditions like Behçet's disease can cause recurrent ulcers in the genital area and may also present with skin lesions elsewhere on the body.

Recommended Treatments
1. Topical Treatments:
- Continue using antiseptic solutions like iodine, but be cautious as they can sometimes irritate the skin further.
- Over-the-counter topical corticosteroids may help reduce inflammation and redness.

2. Oral Medications:
- If a viral infection like herpes is suspected, antiviral medications such as acyclovir can be effective.

- For bacterial infections, antibiotics may be necessary, so consulting a healthcare provider is crucial.

3. Pain Management:
- Non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help manage pain and inflammation.

4. Hygiene Practices:
- Maintain good hygiene in the genital area. Avoid irritants and wear loose-fitting clothing to reduce friction.

5. Consult a Healthcare Provider:
- Given the complexity of your symptoms, including the rash on your shoulders, it is essential to see a healthcare provider for a thorough examination. They may perform tests to determine the exact cause of your ulcers and associated symptoms.

Conclusion
The presence of painful ulcers on the foreskin, along with other systemic symptoms, warrants a careful evaluation by a healthcare professional. It is vital to identify the underlying cause to provide appropriate treatment. While some conditions may resolve with home care, others may require specific medical interventions. If symptoms persist or worsen, do not hesitate to seek medical attention promptly.
