Postoperative swelling after hemorrhoid surgery?
Hello, doctor.
I would like to ask: I had a level 4 hemorrhoid surgery and have been resting at home for nearly 10 days.
Why is the wound still very painful? Also, it started swelling three days after the surgery.
I have been diligently taking sitz baths 4 to 5 times a day, but the pain is still so severe that I can't sleep.
The oral anti-inflammatory and pain medications prescribed by the hospital are ineffective, and I have to go to the emergency room every 2 to 3 days for pain relief injections to alleviate the pain.
During my follow-up appointment after discharge, the doctor said the swelling would gradually subside, but it has been over a week now.
Why is the wound swollen?

Generally, after hemorrhoid surgery, the pain at the surgical site should have significantly decreased within a week.
However, if there are issues such as constipation or diarrhea, it can lead to swelling of the wound.
Additionally, the duration of bowel movements should be minimized, as prolonged straining during defecation is also a contributing factor to wound swelling.

After undergoing hemorrhoid surgery, it is not uncommon to experience significant pain and swelling at the surgical site. Understanding the reasons behind these symptoms can help in managing them effectively.
Firstly, it is essential to recognize that hemorrhoid surgery, particularly for severe cases like grade IV hemorrhoids, involves the removal of swollen tissue and can lead to trauma in the surrounding areas. This trauma can result in inflammation, which is the body’s natural response to injury. Inflammation is characterized by swelling, redness, heat, and pain, and it is a crucial part of the healing process. However, excessive inflammation can lead to prolonged pain and discomfort.
The swelling you are experiencing is likely due to a combination of surgical trauma and the body’s inflammatory response. After surgery, the body sends white blood cells and other substances to the area to begin the healing process, which can cause swelling. This is often exacerbated by factors such as prolonged sitting, inadequate pain management, or infection.
In your case, the fact that you are experiencing persistent pain and swelling even after ten days post-surgery suggests that there may be additional factors at play. Here are some potential reasons for your ongoing symptoms:
1. Infection: One of the most concerning complications after surgery is the risk of infection. If the surgical site becomes infected, it can lead to increased pain, swelling, and possibly fever. Signs of infection may include increased redness, warmth, or discharge from the wound.
2. Hematoma Formation: Sometimes, blood can accumulate in the area where the surgery was performed, leading to a hematoma. This can cause significant swelling and pain. If a hematoma is suspected, it may require drainage.
3. Delayed Healing: Individual healing rates can vary significantly. Factors such as age, overall health, and pre-existing conditions can affect how quickly the body heals. If you have any underlying health issues, this could contribute to slower recovery.
4. Postoperative Pain Management: If the prescribed oral anti-inflammatory and pain medications are not effective, it may be necessary to reassess your pain management plan. In some cases, stronger medications or alternative therapies may be required.
5. Sitting and Activity Levels: After hemorrhoid surgery, it is crucial to avoid prolonged sitting, as this can exacerbate swelling and discomfort. Engaging in gentle movement and avoiding straining during bowel movements can also help alleviate symptoms.
Given that you have been experiencing significant pain and swelling for over a week, it is advisable to follow up with your surgeon or a healthcare provider. They may want to conduct a physical examination and possibly imaging studies to rule out complications such as infection or hematoma.
In the meantime, continue with your sitz baths, as they can help soothe the area and promote healing. Applying ice packs to the area for short periods can also help reduce swelling. Staying hydrated and maintaining a high-fiber diet can prevent constipation and straining during bowel movements, which can worsen pain.
In summary, while some pain and swelling are expected after hemorrhoid surgery, persistent and severe symptoms warrant further evaluation. It is essential to communicate openly with your healthcare provider about your symptoms and any concerns you may have regarding your recovery.
