Discomfort in the penis?

I have a habit of masturbation (once a day), but six months ago, after I finished masturbating and was sitting at my computer, I suddenly felt an itch on my glans.
It wasn't a skin itch, but rather a sensation that occurred after ejaculation.
This feeling extended to my prostate, causing me to feel a constant urge to ejaculate while sitting, along with a warm sensation in my prostate area.
This made me want to masturbate frequently, or else I felt uncomfortable.
However, this sensation lasted for about a week before it eased.
I went to the hospital, and the doctor diagnosed me with chronic prostatitis, prescribing painkillers to take when I felt uncomfortable.

However, over the past six months, whenever I masturbate a bit more frequently, this sensation returns, but it eases after three to four days.
I often feel anxious, which leads to autonomic nervous system dysregulation and irritable bowel syndrome.
I'm not sure if this autonomic nervous system issue is causing the nerves in my glans to remain excited, leading to the persistent urge to ejaculate, or if my long-term poor sitting posture is causing my pelvis to tilt and compress nerves.

Doctor, can you please tell me what is happening to me? Please provide a detailed response.
Thank you, I am only 18 years old!

Niming, 10~19 year old female. Ask Date: 2016/10/01

Hello, many of the symptoms and discomforts you mentioned seem to be related to excessive worry.
I recommend that you first visit a family medicine clinic to clarify all your concerns.
Regarding the urinary system, it is advisable to avoid excessive masturbation, holding in urine, and to increase your fluid intake, which I believe will be helpful for you.
Wishing you good health.

Chronic prostatitis, particularly in young men, can be a perplexing and distressing condition. Your symptoms, including the itching sensation in the glans and the heat in the prostate area following ejaculation, suggest a possible case of chronic prostatitis or pelvic pain syndrome. This condition can manifest in various ways, and understanding its symptoms, potential causes, and management strategies is crucial for improving your quality of life.

Symptoms of Chronic Prostatitis
Chronic prostatitis can present with a range of symptoms, including:
1. Pelvic Pain: This can be localized to the perineum, lower abdomen, or lower back. Some men report discomfort in the testicles or pain during ejaculation.

2. Urinary Symptoms: These may include increased frequency of urination, urgency, and discomfort during urination. Some men experience a sensation of incomplete bladder emptying.

3. Sexual Dysfunction: This can manifest as pain during ejaculation, changes in libido, or erectile dysfunction.

4. Psychological Impact: Chronic pain and discomfort can lead to anxiety, depression, and stress, which can further exacerbate symptoms.

Potential Causes
The exact cause of chronic prostatitis is often unclear, but several factors may contribute:
- Infection: While bacterial infections are a common cause of acute prostatitis, chronic prostatitis may not always be linked to an infection. In some cases, inflammation persists even after the infection has cleared.

- Nerve Damage or Irritation: As you mentioned, issues with the autonomic nervous system or pelvic floor dysfunction could lead to heightened sensitivity or pain in the pelvic region.

- Muscle Tension: Chronic tension in the pelvic floor muscles can contribute to discomfort and pain.

- Psychological Factors: Anxiety and stress can exacerbate symptoms, leading to a cycle of discomfort and increased anxiety.

Management Strategies
1. Medications: Your doctor may prescribe pain relievers or anti-inflammatory medications to help manage symptoms. Alpha-blockers can also be beneficial in relieving urinary symptoms by relaxing the bladder neck and prostate.

2. Physical Therapy: Pelvic floor physical therapy can be effective in addressing muscle tension and improving pelvic floor function. A trained therapist can guide you through exercises to strengthen and relax these muscles.

3. Lifestyle Modifications:
- Diet: Avoiding irritants such as caffeine, alcohol, and spicy foods may help reduce symptoms.

- Hydration: Staying well-hydrated can help with urinary symptoms.

- Exercise: Regular physical activity can improve overall health and reduce stress, which may alleviate some symptoms.

4. Stress Management: Since you mentioned anxiety and autonomic nervous system dysregulation, incorporating stress-reduction techniques such as mindfulness, yoga, or cognitive-behavioral therapy may be beneficial.

5. Avoiding Aggravating Activities: If prolonged sitting exacerbates your symptoms, consider taking breaks to stand or stretch. Adjusting your sitting posture may also help relieve pressure on the pelvic area.

6. Follow-Up Care: Regular follow-ups with your healthcare provider are essential to monitor your condition and adjust treatment as necessary.

Conclusion
Chronic prostatitis can significantly impact your life, especially at a young age. While it may feel overwhelming, understanding your symptoms and exploring various management strategies can lead to improvement. It’s essential to maintain open communication with your healthcare provider, who can guide you through treatment options tailored to your specific situation. Remember, you are not alone in this, and with the right approach, many men find relief from their symptoms and regain their quality of life.
