Foreskin and Glans Health: Common Issues and Concerns - Urology

Prepuce and glans penis

Hello doctor, last week I had a health check-up and the doctor advised me to start retracting my foreskin to clean it, as I didn't know I could do that before.
After a few days of cleaning, I noticed that my foreskin and glans developed 4 to 5 small white bumps that seem to have pus, about the size of a needle.
There are also some bumps on the surrounding foreskin, and the key point is that they hurt when touched.
I'm suspecting it might be a fungal or bacterial issue.
Initially, it was only on one side, but after a day, the other side also developed bumps, and there are some on the foreskin as well.
Could it be that the tap water at home is dirty? I've heard that the water quality in my area is hard, so I'm not sure if something in the water caused these issues.
Do I definitely need to see a doctor?

More Info

Understanding foreskin and glans health is crucial for maintaining overall genital hygiene and preventing potential infections. Based on your description, it sounds like you may be experiencing a condition that could be related to either a fungal or bacterial infection, particularly given the presence of small, painful, white bumps on the glans and foreskin.

Common Issues Related to Foreskin and Glans Health
1. Balanitis: This is an inflammation of the glans (the head of the penis) that can be caused by poor hygiene, infections (bacterial or fungal), or irritants. Symptoms often include redness, swelling, and pain, as well as discharge that may appear white or yellowish.

2. Phimosis: This condition occurs when the foreskin cannot be easily retracted over the glans. It can lead to hygiene issues and increase the risk of infections, including balanitis.

3. Candidiasis (Yeast Infection): Fungal infections, particularly those caused by Candida species, can lead to white patches or bumps on the glans and foreskin. These infections are often associated with itching, burning, and discomfort.

4. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans. They are normal anatomical variations and are not associated with any disease.

5. Sexually Transmitted Infections (STIs): Certain STIs can cause lesions or bumps on the genital area. If you have had unprotected sexual contact, it is important to consider this possibility.

Importance of Medical Evaluation
Given the symptoms you are experiencing—painful bumps, potential discharge, and the rapid development of these symptoms—it is highly advisable to see a healthcare professional. A doctor can perform a physical examination, take a detailed medical history, and possibly conduct tests to determine the exact cause of your symptoms.

Potential Causes of Your Symptoms
- Infection: The presence of white bumps that are painful could indicate an infection, such as a yeast infection or bacterial infection. The fact that the bumps appeared after you started practicing better hygiene may suggest that the area was previously harboring bacteria or fungi that became problematic once exposed.

- Water Quality: While hard water can sometimes irritate the skin, it is unlikely to be the sole cause of an infection. However, if your water quality is poor, it could contribute to skin irritation or exacerbate existing conditions.

Recommendations
1. Avoid Irritants: Until you see a doctor, avoid using any harsh soaps or products on the affected area. Stick to gentle, unscented cleansers.

2. Maintain Hygiene: Continue to practice good hygiene by gently retracting the foreskin and cleaning the area with warm water. Avoid excessive scrubbing, which can irritate the skin further.

3. Seek Medical Attention: It is essential to consult a healthcare provider as soon as possible. They may prescribe antifungal or antibacterial medications depending on the diagnosis.

4. Monitor Symptoms: Keep track of any changes in your symptoms, including the size and number of bumps, any discharge, and whether the pain worsens or improves.

In conclusion, while it is good that you have started to pay attention to your genital hygiene, the symptoms you are experiencing warrant a professional evaluation. Early diagnosis and treatment can prevent complications and ensure better health outcomes.
