Problems with the foreskin and glans?

I am a person with phimosis (the foreskin is long enough that it resembles a sausage with knots at both ends).
Recently, while showering, I noticed a small white bump on my glans (it is slightly raised but not large, resembling a small pimple).
At the same time, I sometimes feel itchiness on the tip of my glans/foreskin, which only subsides during urination.
After noticing this condition, I instinctively paid more attention to the hygiene of my genital area, but there seems to be no improvement.
While writing this, I used my phone's camera and discovered that those small white bumps seem to be ulcerated (red in the center with a white surrounding, resembling atolls); additionally, I found three smaller but similar white bumps at the meatus (also resembling atolls).
Could this be a result of excessive cleaning, or is it balanitis? I must admit that I have had unprotected sexual intercourse, but I have been tested for HIV and syphilis, with negative results (as I had experienced ulcerations on my glans similar to syphilis).
That test was conducted in June of last year, and the unprotected sexual encounter occurred in January of the same year.
Since then, I have used condoms during all sexual activities.
