Glans and foreskin issues?
Hello doctor, recently I have noticed that during sexual activity or masturbation, the sides of my penis shaft are particularly red (there has also been some bleeding, resembling a wound), and there are tiny red spots on the glans that feel especially thin.
It seems that I also have an issue with premature ejaculation.
What should I do about this?

More Info
Understanding glans and foreskin issues is crucial for maintaining sexual health and overall well-being. Based on your description, it sounds like you may be experiencing a combination of symptoms that could be related to irritation, infection, or other underlying conditions affecting the glans (the head of the penis) and the foreskin.
Causes of Glans and Foreskin Issues
1. Irritation or Trauma: Engaging in sexual activity or masturbation can sometimes lead to friction, resulting in irritation or small abrasions on the skin. This can cause redness, tenderness, and even bleeding if the skin is particularly sensitive or if there is excessive force.
2. Infections: Conditions such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can occur due to bacterial or fungal infections. Symptoms may include redness, swelling, itching, and discharge. Yeast infections are also common and can cause similar symptoms.
3. Sexually Transmitted Infections (STIs): Some STIs can present with symptoms affecting the glans and foreskin. For example, herpes can cause painful sores, while other infections may lead to discharge or unusual symptoms.
4. Allergic Reactions: Allergies to soaps, lotions, or latex (from condoms) can cause irritation and redness. It’s important to consider any new products you may have used recently.
5. Skin Conditions: Conditions such as psoriasis or eczema can also affect the genital area, leading to redness and discomfort.
6. Phimosis: If the foreskin cannot be easily retracted, it can lead to irritation and inflammation, particularly during sexual activity.
Solutions and Management
1. Hygiene: Maintain good hygiene by gently washing the area with mild soap and water. Avoid harsh soaps or products that may irritate the skin.
2. Avoid Irritation: If you suspect that friction during sexual activity or masturbation is causing the issue, consider using a water-based lubricant to reduce friction.
3. Topical Treatments: Over-the-counter antifungal creams may help if a yeast infection is suspected. For irritation, a mild hydrocortisone cream can reduce inflammation, but it should be used sparingly and not for prolonged periods without consulting a healthcare provider.
4. Consult a Healthcare Provider: Given the symptoms you described, especially the bleeding and the presence of small red dots, it is essential to consult a healthcare provider or a urologist. They can perform a physical examination, possibly run tests to rule out infections or STIs, and provide a proper diagnosis.
5. Avoid Sexual Activity: Until you have seen a healthcare provider, it may be wise to avoid sexual activity to prevent further irritation or the potential spread of an infection.
6. Discuss Early Ejaculation: If you are experiencing premature ejaculation, this is a separate issue that can often be addressed through behavioral techniques, counseling, or medications. Discuss this with your healthcare provider for tailored advice.
Conclusion
In summary, the symptoms you are experiencing could stem from various causes, including irritation, infections, or skin conditions. It is crucial to seek medical advice to accurately diagnose the issue and receive appropriate treatment. In the meantime, practicing good hygiene, avoiding irritants, and using lubricants can help manage your symptoms. Remember, open communication with your healthcare provider is key to addressing your concerns effectively.
