Glans and foreskin issues?
After more than eight days of masturbation, I started experiencing a slight burning sensation and redness on my penis.
The glans also appeared to have some abrasions (localized erythema), and during that time, I often felt itchy and had some peeling, which persisted even after showering.
Since I am abroad, a local doctor prescribed me Hydrocortisone 1% cream, but after using it for a while, there was no significant improvement.
In September 2016, I returned to Taiwan and visited a urologist, who prescribed Tetracycline HCl ointment and oral antibiotics, but I still felt that the results were not satisfactory.
To this day, the glans still shows signs of similar abrasions, and the color of the skin from the foreskin to the penis appears inflamed and red.
Strangely, even when using condoms during sexual intercourse, I notice a few small red spots on the glans afterward, which disappear after a few days.
These symptoms keep recurring, and I would like to know how to address this issue.

Hello: It is possible that your skin is still healing and has been irritated.
It is recommended to rinse with clean water, ensure it is clean, and then pat dry.
Please keep the area dry regularly.
If this occurs repeatedly, it may be safer to have a blood test.
Thank you for your inquiry.

More Info
Managing persistent glans and foreskin issues can be quite challenging, especially when symptoms like burning, redness, itching, and peeling occur. Based on your description, it seems like you are experiencing a combination of symptoms that could be related to several conditions, including balanitis (inflammation of the glans), posthitis (inflammation of the foreskin), or even a possible allergic reaction or infection.
Symptoms Overview
1. Burning and Redness: These symptoms often indicate inflammation, which can be caused by irritation, infection, or an allergic reaction.
2. Itching and Peeling: This can suggest a fungal infection, such as candidiasis, or a dermatological condition like eczema or psoriasis.
3. Small Red Dots After Intercourse: These could be petechiae (small red or purple spots caused by bleeding under the skin), which may occur due to friction or irritation during sexual activity, especially if there is any underlying inflammation.
Possible Causes
1. Balanitis: This is the inflammation of the glans and can be caused by poor hygiene, infections (bacterial or fungal), or irritants (such as soaps or lotions).
2. Posthitis: Similar to balanitis, but specifically involves the foreskin. It can occur alongside balanitis.
3. Allergic Reactions: Some individuals may react to latex in condoms or to certain lubricants, leading to localized inflammation.
4. Sexually Transmitted Infections (STIs): Certain STIs can present with similar symptoms, so it’s important to rule these out.
Treatment Options
1. Topical Treatments:
- Hydrocortisone Cream: This is a mild steroid that can help reduce inflammation. However, prolonged use can lead to skin thinning, so it should be used cautiously.
- Antibiotic Ointments: Tetracycline HCl ointment is an antibiotic that can help if there is a bacterial infection. If symptoms persist, it may be worth discussing with your doctor about switching to a different antibiotic or considering a culture to identify the specific pathogen.
- Antifungal Creams: If a fungal infection is suspected, topical antifungal treatments (like clotrimazole or miconazole) may be effective.
2. Oral Medications: If topical treatments are not effective, your doctor may prescribe oral antibiotics or antifungals based on the suspected underlying cause.
3. Hygiene Practices:
- Maintain good hygiene by gently washing the area with mild soap and water. Avoid harsh soaps or irritants.
- Ensure the area is thoroughly dried after washing, as moisture can promote fungal growth.
4. Avoid Irritants: If you suspect that condoms or lubricants may be causing irritation, consider switching to hypoallergenic options or using water-based lubricants.
5. Follow-Up with a Specialist: If symptoms persist despite treatment, it may be beneficial to consult a urologist or dermatologist who specializes in genital skin conditions. They may perform tests to rule out STIs or other underlying conditions.
When to Seek Immediate Care
- If you experience severe pain, swelling, or discharge that is unusual.
- If there are systemic symptoms such as fever or chills, which could indicate a more serious infection.
Conclusion
Persistent glans and foreskin issues can be complex and may require a multifaceted approach to treatment. It’s crucial to work closely with healthcare providers to identify the underlying cause and tailor a treatment plan that effectively addresses your symptoms. Keeping a record of your symptoms, triggers, and any treatments you've tried can also be helpful for your healthcare provider in managing your condition.
