Abnormal vaginal discharge

The patient has been experiencing vulvovaginal candidiasis since April of this year.
After consulting various gynecologists, most treatments involved the use of suppositories and medications, with a regimen of inserting medication every three days for two courses.
Although symptoms improved initially, the condition has been recurrent.
After the last consultation with Dr.
Kuo at Chung Shan Medical University Hospital on August 12, the same two-course treatment was prescribed, and Dr.
Kuo indicated that follow-up visits were not necessary.
The patient has since been taking a vaginal-specific probiotic, Femina Care, daily for nearly two months, but still experiences significant vaginal discharge daily, except during menstruation when it is clear.
After menstruation, the discharge returns, especially increasing before and after the menstrual period.
The discharge appears white and curd-like or yellowish, and there is no itching in the vulvar area.
The patient avoids cold foods, wears long pants, does not stay up late, and hand-washes underwear.

The patient is inquiring whether it is necessary to return for a gynecological consultation.
Additionally, they are asking if it is safe to use a vinegar and water solution (1:8) or a povidone-iodine solution (1:20) for douching, and whether this should be done once a day for five minutes.
Thank you for your patience in answering these questions.

Hello, vulvovaginal candidiasis may sometimes recur even after treatment.
If you experience discomfort, please return to the gynecologist for further evaluation.
Self-douching is not recommended.
Wishing you good health.

Understanding abnormal vaginal discharge is crucial for maintaining reproductive health, especially for those experiencing recurrent issues such as yeast infections. Based on your description, it seems you have been dealing with recurrent candidiasis (yeast infections) since April, despite various treatments. This can be frustrating and concerning, especially when the discharge persists even after treatment.

Causes of Abnormal Vaginal Discharge
1. Yeast Infections: These are often characterized by thick, white, cottage cheese-like discharge, which can be accompanied by itching and irritation. However, in some cases, women may experience discharge without significant itching, making it harder to identify the cause.

2. Bacterial Vaginosis (BV): This condition can also cause abnormal discharge, often described as thin and grayish with a fishy odor. It’s important to differentiate between BV and yeast infections, as they require different treatments.

3. Hormonal Changes: Fluctuations in hormones, particularly around menstruation, can lead to changes in discharge. It's common for women to notice increased discharge before and after their menstrual period.

4. Diet and Lifestyle Factors: Certain foods, stress, and lifestyle choices can impact vaginal flora and lead to imbalances, resulting in abnormal discharge.

5. Antibiotic Use: If you have been on antibiotics for other infections, this can disrupt the normal vaginal flora, leading to yeast overgrowth.

Treatment and Management
Given your history of recurrent yeast infections, it may be beneficial to revisit your healthcare provider for a comprehensive evaluation. Here are some steps to consider:
1. Re-evaluation by a Specialist: Since you have already seen multiple gynecologists, it may be worthwhile to seek a second opinion or consult a specialist in infectious diseases or a gynecologist who specializes in recurrent vaginal infections.

2. Testing: A thorough examination, including cultures or swabs, can help identify the specific cause of your discharge. This may include testing for yeast, bacterial vaginosis, and sexually transmitted infections (STIs).

3. Probiotics: While you are currently taking vaginal probiotics, it may take time to see results. Probiotics can help restore the natural balance of bacteria in the vagina, but they are not a quick fix. Consistency is key.

4. Avoiding Irritants: Ensure that you are not using any products that could irritate the vaginal area, such as scented soaps, douches, or feminine hygiene sprays.

5. Home Remedies: Regarding your question about using vinegar or iodine solutions, it’s essential to approach home remedies with caution. While some women find relief with diluted vinegar baths, it’s not universally recommended, as it can sometimes irritate the vaginal mucosa. Always consult with your healthcare provider before trying any home remedies.

6. Dietary Considerations: Maintaining a balanced diet and avoiding excessive sugar can help reduce the risk of yeast infections, as yeast thrives on sugar.

7. Follow-up Care: If your symptoms persist despite these measures, it is crucial to follow up with your healthcare provider. They may suggest alternative treatments or further investigations to rule out other underlying conditions.

Conclusion
In summary, recurrent abnormal vaginal discharge can be a complex issue requiring a multifaceted approach. It is essential to work closely with your healthcare provider to identify the underlying cause and develop an effective treatment plan. Do not hesitate to return for further evaluation if your symptoms continue, as persistent issues can sometimes indicate a need for more specialized care. Your health and comfort are paramount, and addressing these concerns promptly can lead to better outcomes.
