Ultrasound out-of-pocket & fine needle aspiration?

Hello Doctor, I previously asked about the issue of an enlarging breast lump.
I went for a follow-up appointment, and the doctor intended to arrange another ultrasound for me.
However, since I had my last ultrasound on August 8, it hasn't been three months yet, and if I want to have it done now, I would need to pay out of pocket.
I told the doctor that I would think about it, and I would like to ask if I really need to pay for the ultrasound.
Additionally, I would like to inquire about the wound from my previous core needle biopsy, which is located between my two lumps (the ultrasound previously showed both lumps, one being very uniform and the other not so uniform).
Why does the wound not align with the lumps?

Ann, 20~29 year old female. Ask Date: 2016/10/14

Dr. Li Zhonghuai reply Surgery

Hello! It is recommended to bring your original hospital records to another facility to seek assistance.
Regarding the needle insertion point, it will definitely not be directly above the tumor, as the needle can extend 2 centimeters, which poses a risk of puncturing the lung for deeper tumors.
Therefore, the insertion site will be at a distance from the tumor to ensure that the angle of insertion is not too steep.
Wishing you safety! Changhua Hospital cares about you!

When it comes to understanding ultrasound costs and the results of a core needle biopsy, it’s essential to consider both the medical necessity of the procedures and the implications of the findings from your previous examinations.

Understanding Ultrasound Costs
Ultrasound examinations are typically used to visualize internal organs and assess conditions such as tumors, cysts, or other abnormalities. The cost of an ultrasound can vary widely based on several factors, including the type of ultrasound, the facility where it is performed, and whether it is covered by insurance. In your case, since your doctor suggested a follow-up ultrasound but indicated that it would be out-of-pocket due to the timing of your last ultrasound, you should weigh the necessity of the procedure against the cost.

If your doctor believes that the ultrasound is crucial for monitoring the growth of the chest masses, it may be worth considering the expense. Regular monitoring can help detect any changes that may require further intervention. However, if there are no significant changes in your symptoms or if the previous ultrasound results were stable, you might opt to wait until it is covered by insurance or until a more pressing need arises.

Core Needle Biopsy Results
Regarding your core needle biopsy, it’s important to understand the purpose of this procedure. A core needle biopsy is often performed to obtain a tissue sample from a suspicious area, such as a lump or mass, to determine whether it is benign or malignant. The location of the biopsy site relative to the masses you mentioned is also significant.

You noted that the biopsy wound is located between two masses, one of which appeared uniform and the other irregular. This discrepancy can occur for several reasons:
1. Sampling Error: The core needle biopsy may not have sampled the area of concern if the irregular mass was not adequately targeted. This can lead to a situation where the biopsy results do not correlate with the appearance of the masses on ultrasound.

2. Nature of the Masses: The two masses may have different characteristics. The uniform mass could be a benign lesion, while the irregular one may have more complex features that require further evaluation.
3. Healing Process: The healing of the biopsy site can also affect how the surrounding tissues appear on imaging. Swelling or changes in the tissue can sometimes make it difficult to correlate the biopsy site with the masses.

Recommendations
1. Consult Your Doctor: It’s crucial to discuss your concerns with your healthcare provider. They can provide insights into the necessity of the follow-up ultrasound and whether the biopsy results warrant further investigation.

2. Consider Monitoring: If the masses have been stable and there are no new symptoms, your doctor may suggest a watchful waiting approach. However, if there are changes in size or symptoms, further imaging or intervention may be necessary.

3. Follow-Up on Biopsy Results: Ensure that you understand the results of your biopsy. If there are any uncertainties or if the results are inconclusive, your doctor may recommend additional imaging or a repeat biopsy.

In conclusion, while the decision to undergo an additional ultrasound may involve cost considerations, the potential benefits of monitoring your condition should be carefully evaluated. Always prioritize open communication with your healthcare provider to make informed decisions regarding your health.
