Sudden improvement in bowel movements?

I have had difficulty with bowel movements since childhood; there were times when I could go a week without a bowel movement.
However, just before I turned 30, my bowel movements suddenly became regular.
Earlier this year, I underwent hemorrhoidectomy at Tri-Service General Hospital in Neihu, and the doctor advised me to have a colonoscopy afterward.
The surgery was successful, and the doctor did not inform me about the results of the colonoscopy.
Recently, I came across a case of a middle-aged man, around 50 years old, who also experienced a sudden improvement in bowel movements, which then became increasingly narrow, along with a poor appetite.
He was later diagnosed with stage III colorectal cancer after going to the hospital for an examination.
Therefore, I would like to ask the doctor if a sudden improvement in bowel movements is normal.

Dr. Chen Qifeng reply Gastroenterology and Hepatology

The key symptoms in your case are the increasingly narrow stools and loss of appetite, rather than a change from constipation to regular bowel movements.
Additionally, you have undergone a colonoscopy this year with no issues (if there were any problems, the physician would have informed you), so there is no need for concern.

Sudden changes in bowel habits can indeed be a cause for concern, particularly when there is a notable shift from a history of constipation to more regular bowel movements. While many factors can contribute to changes in bowel habits, including dietary changes, increased physical activity, stress, and medical conditions, it is essential to approach these changes with caution, especially given your history of bowel issues and recent surgical intervention.

In your case, the transition from infrequent bowel movements to more regular ones could be attributed to several factors. For instance, after undergoing a hemorrhoidectomy, it is common for individuals to experience changes in bowel habits due to alterations in diet, pain management, and the body's healing process. The surgery may have alleviated some of the discomfort associated with bowel movements, allowing for a more regular schedule. Additionally, if you have made any lifestyle changes, such as increasing fiber intake or physical activity, these could also contribute to improved bowel regularity.

However, the concern arises when changes in bowel habits are accompanied by other symptoms. In the case you mentioned about the 50-year-old man, the combination of increased frequency of bowel movements, changes in stool caliber (becoming thinner), and decreased appetite are red flags that warrant further investigation. These symptoms can be indicative of underlying conditions, including colorectal cancer, particularly if there is a history of constipation followed by a sudden change in bowel habits.

The Bristol Stool Scale can be a helpful tool in assessing stool consistency and shape. Normal stool is typically classified as types 3 and 4, which are sausage-shaped and smooth. If your stools have become consistently thinner or if you notice any changes in color, consistency, or the presence of blood or mucus, these are significant indicators that should prompt further evaluation.

Given your concerns and the potential risk factors associated with colorectal cancer, it would be prudent to consult with a gastroenterologist for a thorough evaluation. This may include a colonoscopy, which is the gold standard for detecting abnormalities in the colon, including polyps or tumors. Additionally, discussing your family history and any other symptoms you may be experiencing will provide your healthcare provider with valuable information to guide their assessment.

In summary, while a sudden change to more regular bowel movements can be a positive development, it is essential to remain vigilant about any accompanying symptoms. Regular monitoring of your bowel habits, maintaining a healthy diet, and seeking medical advice when changes occur are crucial steps in ensuring your gastrointestinal health. If you have any concerns or experience new symptoms, do not hesitate to reach out to your healthcare provider for further evaluation and peace of mind.
