Redness and swelling of the glans and foreskin?
Hello, Dr.
Du.
In July, I had an encounter with a stranger, and I tested negative for HIV using an RTPCR test 14 days later.
Recently, in mid-October (three months later), I tested negative for both HIV antibodies and syphilis.
I had another sexual encounter with a woman recently, during which I used a condom; however, I did not wash off the lubricant properly before leaving.
After about 10 hours, I noticed a greasy feeling and itching in my genital area.
I washed immediately, but the itching persisted the next day, so I used soap to clean the area.
Currently, my genital condition includes redness and swelling of the foreskin, which I can retract fully.
When I touch the inner foreskin, I feel a stinging sensation, and there is a white discharge.
The glans also feels slightly red, with a layer of white, flaky substance on the surface.
I do not experience stinging or burning during urination, but sometimes I have an itchy sensation that makes me feel like I need to urinate.
Overall, there are no unusual signs of conditions like warts, chlamydia, or herpes, and the incubation periods for these infections have passed.
Should I still be concerned that these pathogens are causing my current issues? I would appreciate your help in resolving my concerns.

Based on your situation, it is necessary for a physician to conduct a personal examination.
In the meantime, you should try to keep the foreskin as dry as possible and avoid excessive cleaning (some people use soap or body wash too frequently, which can damage the skin and make it look worse).
It is safer to see a doctor when you have the opportunity.

More Info
Hello, thank you for your inquiry regarding the redness and swelling of the glans. Based on your description, it seems you are experiencing some discomfort and changes in the appearance of your genital area, particularly after recent sexual encounters. Let's break down your concerns and provide some insights.
1. Symptoms Overview: You mentioned experiencing a greasy feeling, itching, and redness of the glans and foreskin, along with the presence of a white discharge that resembles peeling skin. These symptoms can be indicative of several conditions, including irritation, infection, or an allergic reaction to products such as lubricants.
2. Possible Causes:
- Irritation: The use of lubricants, especially if not properly washed off, can lead to irritation of the sensitive skin of the genital area. This can cause redness, itching, and a greasy feeling.
- Infection: The symptoms you describe could also suggest a yeast infection or a bacterial infection. Yeast infections often present with itching, redness, and a white discharge. Bacterial infections can also cause similar symptoms.
- Sexually Transmitted Infections (STIs): While you have tested negative for HIV and syphilis, other STIs such as herpes or human papillomavirus (HPV) can cause symptoms like yours. However, you mentioned that the symptoms do not resemble those typically associated with these infections, which is reassuring.
3. When to Seek Medical Attention: Since you are experiencing persistent symptoms such as itching and discomfort, it would be advisable to consult a healthcare provider. They can perform a physical examination and possibly run tests to rule out infections or other conditions. It's important to address these symptoms promptly to prevent any potential complications.
4. Self-Care Recommendations:
- Hygiene: Ensure that you maintain good hygiene by washing the genital area gently with mild soap and water. Avoid using harsh soaps or products that may further irritate the skin.
- Avoid Irritants: If you suspect that the lubricant may have caused irritation, consider switching to a different product or using a water-based lubricant that is less likely to cause irritation.
- Avoid Sexual Activity: Until you have a clearer understanding of your symptoms, it may be wise to avoid sexual activity to prevent further irritation or potential transmission of infections.
5. Follow-Up Testing: If your symptoms persist or worsen, follow-up testing may be necessary. This could include swabs for bacterial or fungal infections, as well as further STI testing if deemed appropriate by your healthcare provider.
In conclusion, while your symptoms may be related to irritation from lubricant use, it is essential to consult with a healthcare professional for an accurate diagnosis and appropriate treatment. Early intervention can help alleviate your symptoms and prevent any potential complications. Thank you for reaching out, and I hope you find relief soon!
