Chest issues

Hello, doctor.
For the past three days, I've been experiencing pain in the area just below my right breast, around the location of what seems to be the second-to-last rib.
The pain is more noticeable when I take deep breaths or straighten my chest.
Today, the pain has become more generalized but is not as intense as it was in the previous two days.
What could be causing this? (I have also been experiencing some acid reflux, stomach cramps, and general gastric discomfort recently, and I am taking medication for my stomach.)

Xiao Hong, 20~29 year old female. Ask Date: 2016/11/12

Dr. Hong Yuanbin reply Internal Medicine

Hello: Based on your symptom description, it could be a gastric issue, pleuritis, or a muscle strain.
Further evaluation with a chest X-ray and gastrointestinal endoscopy may be needed for a more accurate diagnosis.
Thank you for your inquiry.

Chest pain can be a concerning symptom, and understanding its potential causes is crucial for determining when to seek medical help. Based on your description, the pain you are experiencing in the right lower chest area, particularly when taking deep breaths or straightening your posture, could be attributed to several factors.

1. Musculoskeletal Issues: One of the most common causes of localized chest pain, especially when it worsens with movement or deep breathing, is musculoskeletal strain. This could be due to muscle strain, rib injury, or inflammation of the cartilage connecting the ribs to the sternum (costochondritis). If you have been engaging in activities that involve heavy lifting or awkward movements, this could be a likely cause.

2. Gastrointestinal Problems: Given your history of acid reflux, stomach spasms, and gastrointestinal discomfort, it is possible that your chest pain is related to these issues. Conditions like gastroesophageal reflux disease (GERD) can cause pain that mimics cardiac pain, often exacerbated by certain positions or movements. The pain may also be referred from the stomach or esophagus, particularly if you are experiencing symptoms like heartburn or indigestion.

3. Pulmonary Causes: Although less likely given your description, it is essential to consider pulmonary issues such as pleuritis (inflammation of the lining of the lungs) or even a pulmonary embolism, especially if you have risk factors such as prolonged immobility or a history of clotting disorders. These conditions typically present with sharp pain that worsens with deep breaths.

4. Cardiac Considerations: While your symptoms may not strongly suggest a cardiac issue, it is important to rule out any heart-related problems, especially if you have risk factors such as a family history of heart disease, high blood pressure, or high cholesterol. Cardiac pain often presents as pressure or tightness rather than sharp pain and may be accompanied by other symptoms like shortness of breath, sweating, or nausea.

When to Seek Help
You should seek medical attention if:
- The pain is severe or worsening.

- You experience additional symptoms such as shortness of breath, dizziness, or sweating.

- The pain persists or is accompanied by other concerning symptoms like nausea or vomiting.

- You have risk factors for heart disease or a history of significant gastrointestinal issues.

Recommendations
Given your current symptoms and history of gastrointestinal discomfort, it may be beneficial to:
- Continue taking your prescribed medications for gastric issues, as they may help alleviate some of the discomfort.

- Avoid activities that exacerbate the pain, such as heavy lifting or twisting motions.

- Monitor your symptoms closely. If the pain persists or worsens, or if you develop new symptoms, consider visiting a healthcare provider for a thorough evaluation.

In summary, while your symptoms may be related to musculoskeletal or gastrointestinal issues, it is essential to remain vigilant and seek medical advice if your condition does not improve or if you experience any alarming symptoms. Always prioritize your health and well-being, and do not hesitate to consult a healthcare professional for personalized advice and treatment.
