Pain in the lower right side of the body?

Since August, I have been experiencing soreness and numbness from my lower right back, abdomen, waist, and hips down to the soles of my feet.
I visited a gynecologist (one doctor mentioned a small growth on my right ovary, while another said I have a uterine fibroid on the left side), a rehabilitation specialist (who couldn't find any issues and just recommended physical therapy), and an orthopedic doctor (who took X-rays and found no bone problems, suggesting it might be piriformis syndrome; I took medication and paid out of pocket for laser treatment, but the issue persists).
I'm unsure which specialist to consult next.
In June and July, due to the hot summer, I slept with a fan blowing directly on my body, particularly on the right side, without covering myself with a blanket.
Especially during my menstrual period, my abdomen feels very bloated, and the discomfort in my lower right side worsens, with numbness in my feet.
I would like to know if this could be causing the soreness and numbness in my lower right side? Could it be due to poor blood circulation? (I really can't figure out the cause of these symptoms.)

Qian, 30~39 year old female. Ask Date: 2016/11/17

Dr. Yan Sumei reply Traditional Chinese Medicine

Dear Netizen Qian: Based on your description, it makes sense that sleeping with a fan blowing directly on your body could lead to such symptoms.
I recommend consuming warm foods (such as lamb stew, rice wine chicken, or ginger duck) to help alleviate your symptoms.
Wishing you health and safety! Sincerely, Yen Su-mei, Department of Traditional Chinese Medicine, Nantou Hospital.

More Info

Understanding right lower body pain can be quite complex, especially when it involves multiple areas such as the lower back, abdomen, hips, and feet. Based on your description, it seems that you have consulted various specialists, including gynecologists, rehabilitation doctors, and orthopedic surgeons, but have not yet found a definitive diagnosis or effective treatment. Let's break down the potential causes and treatment options for your symptoms.

Potential Causes of Right Lower Body Pain
1. Gynecological Issues: You mentioned that one doctor found a small growth on your right ovary and another noted a fibroid on your left uterus. Both conditions can lead to referred pain in the lower back and legs. Ovarian cysts, in particular, can cause discomfort, especially during menstruation, and may lead to symptoms like bloating and pain in the lower abdomen and back.

2. Musculoskeletal Problems: The mention of piriformis syndrome suggests that the muscle may be compressing the sciatic nerve, leading to pain and numbness that radiates down the leg. This can be exacerbated by prolonged sitting or specific sleeping positions, such as having a fan blowing directly on one side of your body, which may lead to muscle tension or spasms.

3. Nerve Compression or Circulation Issues: If you are experiencing numbness in your feet, it could indicate nerve compression or issues with blood circulation. Conditions like sciatica, which involves irritation of the sciatic nerve, can cause pain that radiates from the lower back down to the legs and feet.

4. Inflammatory Conditions: Chronic pain can also be a sign of underlying inflammatory conditions, such as arthritis or fibromyalgia, which can cause widespread pain and discomfort.

Treatment Options
1. Gynecological Follow-Up: It is essential to follow up with a gynecologist regarding the ovarian cyst and fibroid. They may recommend monitoring the cyst or fibroid or suggest treatment options if they are causing significant symptoms.

2. Physical Therapy: Since you have already seen a rehabilitation doctor, continuing physical therapy may help. A physical therapist can provide targeted exercises to strengthen the muscles around the pelvis and lower back, which may alleviate some of the pain associated with piriformis syndrome or other musculoskeletal issues.

3. Pain Management: Over-the-counter pain relievers like NSAIDs (e.g., ibuprofen) can help manage inflammation and pain. If the pain persists, a doctor may prescribe stronger medications or recommend injections to reduce inflammation.

4. Lifestyle Modifications: Consider adjusting your sleeping position and avoiding direct airflow from fans on your body, especially during the night. Ensure you are using proper ergonomics when sitting or standing for extended periods.

5. Alternative Therapies: Some patients find relief through acupuncture, chiropractic care, or massage therapy. These treatments can help relieve muscle tension and improve circulation.

6. Diagnostic Imaging: If symptoms persist despite treatment, further imaging studies such as MRI or CT scans may be warranted to rule out any structural issues in the spine or pelvis.

Conclusion
Your symptoms are multifaceted, and it is crucial to approach them holistically. While it is possible that your sleeping habits and environmental factors contribute to your discomfort, underlying medical conditions should not be overlooked. It may be beneficial to seek a multidisciplinary approach, involving gynecologists, physical therapists, and pain management specialists, to address the various aspects of your pain. Keeping a symptom diary may also help your healthcare providers identify patterns and triggers related to your pain.
