Glans and foreskin issues?
Hello, doctor.
I have some questions: 1.
I have a dark line at the tip of my glans; is this normal? 2.
When I retract my foreskin completely, there are small openings between the glans and the foreskin, and smegma accumulates in there.
Is this normal?

Dr. Sun Hongshun reply Urology
Hello: I don't quite understand your question.
Regarding smegma, it is recommended to rinse with warm water appropriately and avoid excessive cleaning.
If you still have questions, please consult a urologist.

Understanding glans and foreskin issues is essential for maintaining good genital health. Let's address your concerns one by one.
1. Dark Line on the Glans: The appearance of a dark line on the tip of the glans (the head of the penis) can be a common concern. In many cases, this could be a normal anatomical variation. The skin of the glans can sometimes appear darker due to increased pigmentation, which is not uncommon. However, if this line is new, changing in appearance, or accompanied by other symptoms such as pain, itching, or discharge, it would be advisable to consult a healthcare professional for a thorough examination. Conditions like pearly penile papules, which are small, harmless bumps, can also appear on the glans and are often mistaken for something more serious.
2. Space Between the Foreskin and Glans: The presence of a gap or "hole" between the retracted foreskin and the glans can be a normal anatomical feature, especially in individuals who are uncircumcised. This space is often referred to as the "meatal opening" and is where the urethra opens. The accumulation of smegma, which is a combination of dead skin cells, oils, and other bodily secretions, is also common in this area, particularly if the foreskin is not retracted regularly for cleaning. It is important to maintain good hygiene by gently cleaning the area to prevent any buildup that could lead to irritation or infection.
Additional Considerations:
- Hygiene: Regular cleaning of the glans and the inner foreskin is crucial. Smegma can accumulate if not cleaned properly, leading to potential infections or conditions like balanitis, which is inflammation of the glans.
- Foreskin Retraction: If you are able to retract the foreskin fully without pain, it is generally a good sign. However, if you experience discomfort or if the foreskin cannot be retracted easily, this could indicate a condition known as phimosis, where the foreskin cannot be pulled back over the glans. This condition can lead to hygiene issues and may require medical attention.
- Changes in Appearance: Any sudden changes in the appearance of the glans or foreskin, such as new growths, discoloration, or persistent irritation, should be evaluated by a healthcare provider.
- Sexual Health: If you are sexually active, it is important to be aware of sexually transmitted infections (STIs) that can affect the genital area. Regular check-ups and practicing safe sex can help mitigate risks.
In summary, while some variations in the appearance of the glans and foreskin can be normal, it is always best to consult with a healthcare professional if you have any concerns or if you notice changes that cause discomfort or anxiety. They can provide a proper diagnosis and recommend any necessary treatment or management strategies. Regular check-ups and open discussions about genital health are vital for overall well-being.
