Lower back and hip pain?
Hello Doctor, I am experiencing soreness and pain in my left hip and lower back.
Currently, I can only sleep on my side (the right side).
If I lie flat, my hip becomes very sore, radiating down to my calf, which also feels painful.
Lying on my stomach also causes pain, and sometimes walking is painful as well.
During painful episodes, I also feel discomfort in my thigh and calf.
My left leg cannot fully relax; as soon as I try to relax it, I experience pain, and my lower back pain prevents me from standing up straight.
I have consulted a traditional Chinese medicine practitioner, who mentioned that there is a protrusion in my lower back (near the left hip area).
Although there has been significant relief, my sleeping situation has not improved; the only change is that my lower back can now be straightened.
I would like to ask the doctor if this could be sciatica or another condition.
Are there any rehabilitation exercises or movements that could help alleviate the pain? What should I pay attention to? Thank you.

Dr. Shi Guozheng reply Orthopedics
It could be sciatica, which is caused by a herniated disc or bone spurs in the spine that compress the sciatic nerve, resulting in pain.
The symptoms can radiate from the back down to the buttocks, the back of the thigh, and into the calf and foot.
Sometimes, there may be a sensation of numbness, and in severe cases, there can be weakness in the foot.
It is advisable to consult a physician for an evaluation.

More Info
Understanding lower back and hip pain can be quite complex, as there are numerous potential causes and contributing factors. Based on your description of pain in your left hip and lower back, which worsens with certain positions and activities, it is essential to consider several possibilities.
Potential Causes of Your Pain
1. Sciatica: Given your symptoms, one possibility is sciatica, which occurs when the sciatic nerve is compressed or irritated. This can lead to pain that radiates from the lower back down through the hip and into the leg. Symptoms often include sharp pain, numbness, or tingling sensations.
2. Herniated Disc: A herniated disc in the lumbar spine can also cause similar symptoms. If the disc protrudes, it can press on nearby nerves, leading to pain that may radiate down the leg.
3. Muscle Strain or Ligament Sprain: Overexertion or improper lifting techniques can lead to muscle strains or ligament sprains in the lower back, which can cause localized pain and discomfort.
4. Hip Joint Issues: Conditions such as hip osteoarthritis or labral tears can lead to pain in the hip area, which may also refer pain to the lower back and leg.
5. Piriformis Syndrome: This condition occurs when the piriformis muscle, located in the buttock region, spasms and irritates the sciatic nerve, leading to pain that can mimic sciatica.
Relief Strategies
1. Physical Therapy: Engaging in physical therapy can be highly beneficial. A physical therapist can design a personalized exercise program that focuses on strengthening the muscles around your spine and hips, improving flexibility, and correcting posture. Specific exercises may include stretches for the hip flexors, hamstrings, and lower back, as well as strengthening exercises for the core and glutes.
2. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and reduce inflammation. However, it is essential to consult with a healthcare provider before starting any medication.
3. Heat and Cold Therapy: Applying heat can help relax tight muscles, while cold packs can reduce inflammation. Alternating between heat and cold can provide relief.
4. Posture and Ergonomics: Pay attention to your posture, especially when sitting or sleeping. Using supportive pillows and maintaining a neutral spine can help alleviate pressure on your lower back and hips.
5. Activity Modification: Avoid activities that exacerbate your pain. If certain movements or positions trigger discomfort, try to modify them or take breaks to prevent further strain.
6. Mind-Body Techniques: Practices such as yoga, tai chi, or mindfulness meditation can help manage pain and improve overall well-being. These techniques can also enhance flexibility and strength.
When to Seek Further Evaluation
If your symptoms persist despite conservative management, or if you experience severe pain, numbness, or weakness in your legs, it is crucial to seek further evaluation from a healthcare professional. Imaging studies such as X-rays or MRI may be necessary to identify any underlying structural issues.
Conclusion
In summary, your symptoms may stem from various conditions, including sciatica, herniated discs, muscle strains, or hip joint issues. A comprehensive approach involving physical therapy, pain management, and lifestyle modifications can help alleviate your discomfort. Always consult with a healthcare provider for a thorough assessment and tailored treatment plan. Remember, early intervention can lead to better outcomes and improved quality of life.
