Persistent Gurgling Sounds in Left Abdomen: What Could Be Wrong? - Gastroenterology and Hepatology

For over a year, there has been unusual sounds coming from the left side of the abdomen?

Hello, Doctor.
I am currently a senior in high school and preparing for the university entrance exam in January.
For the past year, I have been troubled by a persistent issue: I often hear gurgling sounds from the lower left side of my abdomen.
If I intentionally practice diaphragmatic breathing, the moment I inhale, my left abdomen makes loud gurgling noises.
When I exhale and press down with my hand, the same sound occurs.
This also happens when I exert pressure on my abdomen, but there is no pain or other sensations—just the loud gurgling.
This situation tends to occur when I am slightly hungry (even before I feel hungry).
My eating habits are not very regular; I have a normal appetite but digest food quickly.
Additionally, as a member of the school sports team, I have a high level of physical activity, so I eat small meals throughout the day.
I am relatively thin, measuring 168 cm and weighing 54 kg.
I also enjoy snacks (about three to five chocolate cookies a day).
Occasionally, I experience bloating, abdominal pain, and diarrhea (which was more frequent during middle school but has decreased now).
I also tend to slouch while sitting (not sure if this is related).
Initially, I thought the symptoms might be due to excess air that would eventually pass, but it has been almost a year now, and the situation has not improved.
I am a bit worried.
Doctor, what could be the issue? Can I manage this on my own, or do I need to seek medical attention? Thank you, Doctor!

Hello: The phenomenon of borborygmi, if not accompanied by cramping or constipation, is not a concern.
If you experience difficulty in bowel movements, you can increase your intake of water and fruits and vegetables, and ensure daily bowel movements to prevent the overgrowth of gas-producing bacteria.

The persistent gurgling sounds in your left abdomen can be attributed to several factors, and while it may seem benign, it is essential to understand the underlying causes and when to seek medical attention.
Firstly, the sounds you are experiencing, often described as "gurgling," are typically related to the movement of gas and fluids through the intestines. This phenomenon is known as "borborygmi," which is a normal occurrence in the digestive process. However, when these sounds become persistent or are accompanied by other symptoms, it may indicate an underlying issue.

Given your description, several factors could be contributing to the gurgling sounds:
1. Dietary Habits: You mentioned that your eating habits are not entirely regular, and you consume small meals frequently throughout the day. This can lead to increased gas production, especially if your meals are high in carbohydrates or sugars, which can ferment in the gut. Additionally, consuming snacks like chocolate cookies can contribute to digestive irregularities, particularly if they are high in sugar and low in fiber.

2. Physical Activity: As an athlete, your increased physical activity can also influence your digestive system. Intense exercise can stimulate the intestines, leading to increased motility and gas production. This could explain why you notice the sounds more when you are breathing deeply or exerting pressure on your abdomen.

3. Gastrointestinal Sensitivity: Some individuals have more sensitive gastrointestinal tracts, which can lead to heightened awareness of normal digestive sounds. If you have a history of gastrointestinal issues, such as bloating or diarrhea, it may be worth considering if you have a mild form of irritable bowel syndrome (IBS) or food intolerances.

4. Posture: You mentioned having a habit of slouching or hunching over. Poor posture can affect how your digestive organs function and may contribute to discomfort or unusual sounds in the abdomen. Maintaining good posture can help alleviate some of these symptoms.

5. Air Swallowing: If you are eating quickly or talking while eating, you may be swallowing air, which can lead to increased gas in the intestines and contribute to the gurgling sounds.

While the absence of pain or other concerning symptoms is a positive sign, it is still advisable to monitor your situation closely. Here are some recommendations:
- Dietary Adjustments: Consider keeping a food diary to identify any potential triggers for your symptoms. Increasing your fiber intake and reducing high-sugar snacks may help improve your digestive health.

- Hydration: Ensure you are drinking enough water throughout the day, as proper hydration is crucial for digestion.

- Posture Awareness: Work on maintaining good posture, especially while sitting, to reduce any potential pressure on your abdomen.

- Consultation: If the gurgling persists or worsens, or if you begin to experience additional symptoms such as pain, significant changes in bowel habits, or weight loss, it would be wise to consult a healthcare professional. They may recommend further evaluation, such as imaging studies or gastrointestinal assessments, to rule out any underlying conditions.

In conclusion, while the gurgling sounds you are experiencing may be related to normal digestive processes, it is essential to remain vigilant and proactive about your health. Making dietary and lifestyle adjustments can often alleviate symptoms, but do not hesitate to seek medical advice if you have concerns. Your health is paramount, especially as you prepare for important examinations in the coming months.
