Left lower abdominal issues?

Doctor, I would like to ask you something.
As an athlete, I've noticed that whenever I perform leg raises or running movements during my home workouts, I hear a gurgling sound in my lower left abdomen.
What could be the issue? Could it be related to my recent bowel movements being a bit dark green?

Xiao Lan, 10~19 year old female. Ask Date: 2021/06/01

Dr. Chen Shidian reply Gastroenterology and Hepatology

It may be the sound of gastrointestinal peristalsis; further observation is recommended.

Understanding the sounds and sensations in the left lower abdomen, especially in the context of physical activity, can be quite concerning. The "water sounds" or gurgling noises you are experiencing are likely related to the normal functioning of your gastrointestinal (GI) tract. These sounds, known as borborygmi, are produced by the movement of gas and fluids in the intestines. They can become more pronounced during physical activity, particularly when engaging in movements that involve the core muscles, such as leg raises or running.

The presence of these sounds does not necessarily indicate a problem, especially if they are accompanied by normal bowel movements and no significant pain or discomfort. However, you mentioned that your stool has been somewhat greenish in color. This can occur for several reasons, including dietary changes, the consumption of certain foods (like leafy greens or foods with artificial coloring), or even rapid transit through the intestines, which can prevent bile from breaking down completely. Green stool is often not a cause for concern unless it is accompanied by other symptoms such as diarrhea, abdominal pain, or blood in the stool.

Given your background as an athlete, it's also important to consider the impact of your diet and hydration on your digestive health. Dehydration or changes in your diet can lead to variations in bowel habits and stool consistency. If you are consuming a high-fiber diet, it may also contribute to changes in stool color and the sounds you hear in your abdomen.

However, if the gurgling sounds are accompanied by persistent pain, discomfort, or changes in your bowel habits, it may warrant further investigation. Conditions such as irritable bowel syndrome (IBS), gastrointestinal infections, or even structural issues like diverticulitis could potentially cause similar symptoms. Since you have a history of abdominal discomfort and have undergone various examinations, it may be beneficial to keep a detailed record of your symptoms, including when they occur, their intensity, and any associated factors (like food intake or physical activity). This information can be valuable for your healthcare provider in determining if further diagnostic tests, such as imaging studies or endoscopy, are necessary.

In summary, while the sounds you are hearing in your left lower abdomen during physical activity are likely normal and related to gastrointestinal function, it is essential to monitor your overall symptoms. If you experience any significant changes or additional symptoms, consulting with a healthcare professional for a comprehensive evaluation would be advisable. Maintaining a balanced diet, staying hydrated, and being mindful of your body's responses during exercise can also contribute positively to your digestive health.
