There is something unusual in the lower left abdomen?

Hello Dr.
Chen: I would like to ask about a recent strange sensation in my lower left abdomen.
It's not exactly painful, but I feel a pressure sensation that sometimes feels warm or cool (it's hard to describe).
It's not constant; it seems to be intermittent.
The area from my navel to about 10 centimeters up towards my left flank feels a bit tight, but it's not sore or painful (there's no pain when I press on it).
I haven't experienced diarrhea, and my stool color is normal.
I have visited a urologist, and tests including urine analysis, X-rays, and ultrasound showed no stones; everything was normal.
I don't have a frequent urge to urinate, and urination is not painful.
My genital area is also not painful.
At night, I can sleep soundly until morning without any issues.
I'm unsure what might be wrong.
Thank you for your help, doctor.

Understanding unusual sensations in the left lower abdomen can be quite complex, as there are numerous potential causes for such symptoms. Based on your description, it seems you are experiencing a combination of sensations that include a feeling of pressure, warmth, and occasional tightness, but without significant pain or gastrointestinal disturbances. This can be indicative of several underlying issues, which I will explore further.

Firstly, the left lower abdomen is primarily associated with the sigmoid colon, part of the intestines, as well as the left ovary and fallopian tube in females. Given that you have already undergone a thorough evaluation by a urologist, including urine tests and imaging studies like X-rays and ultrasounds, it is reassuring that no significant abnormalities were found in those areas.

One possible explanation for your symptoms could be related to gastrointestinal issues, such as irritable bowel syndrome (IBS). IBS can manifest as abdominal discomfort, bloating, and changes in bowel habits, but it does not always present with diarrhea or constipation. The pressure and tightness you describe could be related to gas buildup or spasms in the intestines. It is important to note that stress and dietary factors can exacerbate these symptoms, so keeping a food diary and noting any correlations with your symptoms may be beneficial.

Another consideration is the possibility of musculoskeletal issues. Sometimes, tightness or discomfort in the abdominal area can arise from muscle strain or tension, especially if you have been engaging in activities that involve bending or lifting. This type of discomfort may not present as classic pain but rather as a sensation of tightness or pressure. Gentle stretching and relaxation techniques may help alleviate these symptoms.

Additionally, since you mentioned that the sensations are not constant and do not interfere with your sleep or urinary function, it may be worthwhile to consider the role of anxiety or stress. Psychological factors can manifest physically, leading to sensations that may feel unusual or uncomfortable. If this resonates with you, exploring stress management techniques or speaking with a mental health professional could be beneficial.

Given that you have ruled out significant urological issues, it may also be prudent to follow up with a gastroenterologist if your symptoms persist or worsen. They may recommend further investigations, such as a colonoscopy or specialized tests to evaluate for conditions like diverticulitis or other gastrointestinal disorders that may not have been apparent in initial evaluations.

In summary, while your symptoms may be concerning, they do not appear to indicate an acute medical emergency. It is essential to continue monitoring your symptoms and maintain open communication with your healthcare providers. Keeping track of any changes, triggers, or patterns in your symptoms can provide valuable information for further evaluation. If you experience any new symptoms, such as significant pain, changes in bowel habits, or other concerning signs, do not hesitate to seek medical attention promptly.
