Sensitivity of the glans, with a prickling sensation upon contact?
Hello Doctor: I usually have a condition where my glans is covered by the foreskin.
When I have an erection, it can normally be exposed, and I can fully wash it during a shower without any issues.
However, when my glans is exposed, it is very sensitive, and I experience a stinging sensation when it rubs against my underwear, which is uncomfortable.
Therefore, during masturbation, I only rub the area behind the glans with the foreskin.
If I accidentally rub the glans, it becomes extremely uncomfortable.
I have read online that sensitivity of the glans can lead to premature ejaculation, but in my case, I find it difficult to ejaculate if I touch the glans.
How can I address this issue? Additionally, my glans seems to have a color that is somewhat between pink and purple, is this a normal phenomenon? Thank you, Doctor, for your time, and I look forward to your response.

Hello: Your situation is quite common.
It is recommended to keep the glans exposed as much as possible to reduce sensitivity.
However, if retracting the foreskin causes tightness, you should consult a physician for examination.
As for the color of the foreskin, it varies from person to person; some may have a darker shade, some lighter, and others may have a mottled appearance.
There's no need to worry too much.
Thank you for your inquiry.

Dear Patient,
Thank you for reaching out with your concerns regarding sensitivity in the glans (the head of the penis). It’s important to address these issues, as they can significantly impact your comfort and sexual health.
From your description, it seems that you experience heightened sensitivity in the glans, particularly when it comes into contact with clothing or during sexual activities. This sensitivity can be attributed to several factors, including the presence of the foreskin, which can affect the exposure and sensitivity of the glans over time. When the glans is covered by the foreskin, it is less exposed to friction and environmental stimuli, which can lead to increased sensitivity once it is exposed.
In terms of the discomfort you experience, it’s not uncommon for individuals to have varying levels of sensitivity in the glans. Some men may find that their glans is overly sensitive, leading to discomfort during activities such as masturbation or sexual intercourse. This heightened sensitivity can sometimes be linked to a condition known as "hyperesthesia," where the nerve endings in the area are more reactive than usual.
Regarding your concern about premature ejaculation (PE), it’s important to note that while increased sensitivity can sometimes contribute to PE, it can also lead to the opposite effect, as you mentioned. Many men with heightened sensitivity may find it difficult to achieve ejaculation due to discomfort or pain when the glans is stimulated.
To address your concerns, here are some potential solutions:
1. Desensitizing Creams: Over-the-counter desensitizing creams or sprays can help reduce sensitivity in the glans. These products typically contain lidocaine or benzocaine, which can numb the area slightly and may help you feel more comfortable during sexual activities.
2. Gradual Exposure: If you are comfortable, gradually exposing the glans to different stimuli can help desensitize the area over time. This could involve gentle rubbing or using different textures to acclimate the glans to stimulation.
3. Consultation with a Specialist: Since you mentioned that your glans has a different coloration, it may be beneficial to consult a urologist or dermatologist. They can assess whether the coloration is within the normal range or if it indicates an underlying condition. Variations in color can be normal, but it’s always best to have a professional evaluation to rule out any concerns.
4. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises such as Kegels can help improve control over ejaculation and enhance sexual function. This can be particularly beneficial if you experience difficulty ejaculating.
5. Communication with Partners: If you are sexually active, communicating with your partner about your sensitivity can help create a more comfortable experience for both of you. Adjusting techniques or positions may alleviate discomfort.
6. Avoiding Irritants: Ensure that any soaps or detergents used in the genital area are mild and free from irritants. Sometimes, harsh chemicals can exacerbate sensitivity.
In conclusion, sensitivity in the glans can be a common issue, and there are several strategies you can employ to manage it. It’s crucial to listen to your body and seek professional advice if discomfort persists or worsens. Remember, sexual health is an important aspect of overall well-being, and addressing these concerns can lead to a more satisfying experience.
Best wishes for your health and comfort, and I hope you find relief soon.
Sincerely,
