Spots on the foreskin?
Recently, I have noticed black spots at the tip of the penis near the glans and at the base of the penis, along with what appears to be scabbing.
There is also itching and pain, and I am unsure what this condition might be.

Hello: Your issue needs to be evaluated by a urologist at the hospital to determine the cause.
Please keep the area dry, maintain a light diet, and drink plenty of fluids (after washing with plain water, dry the area and avoid using soap or body wash for the time being).
It is advisable to visit the hospital for further assessment.

More Info
When it comes to spots on the penis, particularly those that appear as black moles or crusty lesions, there are several potential causes that should be considered. It is essential to approach this issue with caution, as various conditions can lead to similar symptoms, and an accurate diagnosis is crucial for appropriate treatment.
Possible Causes
1. Pearly Penile Papules: These are small, harmless bumps that can appear around the glans (tip) of the penis. They are often mistaken for warts or other lesions but are completely benign and do not require treatment.
2. Seborrheic Keratosis: This is a common skin condition that can appear as brown, black, or tan spots on the skin, including the genital area. These are generally harmless and do not require treatment unless they become bothersome.
3. Moles (Nevi): Just like on other parts of the body, moles can appear on the penis. If they change in size, color, or shape, or if they become painful or itchy, it is important to have them evaluated by a healthcare provider.
4. Genital Warts: Caused by the human papillomavirus (HPV), these can appear as small, flesh-colored or gray growths and may itch or cause discomfort. They can be treated with topical medications or procedures to remove them.
5. Fungal Infections: Conditions like candidiasis (yeast infection) can cause itching, redness, and the appearance of crusty lesions. Treatment typically involves antifungal medications.
6. Balanitis: This is inflammation of the glans and can be caused by infections, poor hygiene, or irritants. Symptoms may include redness, itching, and the formation of crusty lesions.
7. Skin Conditions: Conditions such as psoriasis or eczema can also affect the genital area, leading to itchy, scaly patches.
8. Sexually Transmitted Infections (STIs): Certain STIs can cause lesions or spots on the genital area. If there is a possibility of exposure, it is crucial to get tested.
Treatment Options
The treatment for spots on the penis largely depends on the underlying cause. Here are some general approaches:
- Observation: If the spots are benign (like pearly penile papules or seborrheic keratosis), no treatment may be necessary. Regular monitoring for any changes is advised.
- Topical Treatments: For conditions like fungal infections or genital warts, topical antifungal or antiviral medications may be prescribed.
- Cryotherapy or Laser Treatment: For warts or other lesions that are bothersome, procedures such as cryotherapy (freezing) or laser treatment can be effective.
- Surgical Removal: If a mole or lesion is suspicious or bothersome, a healthcare provider may recommend a biopsy or surgical removal.
- Good Hygiene: Maintaining proper hygiene can help prevent infections and irritations that may lead to spots or lesions.
When to Seek Medical Attention
It is crucial to consult a healthcare provider if you experience any of the following:
- Persistent pain or itching
- Changes in the appearance of the spots (size, color, shape)
- Bleeding or discharge from the lesions
- Any new lesions that appear suddenly
- Symptoms that do not improve with over-the-counter treatments
In conclusion, while spots on the penis can be concerning, many are benign and easily treatable. However, due to the sensitive nature of the area and the potential for serious conditions, it is always best to seek professional medical advice for an accurate diagnosis and appropriate treatment plan.
