Recurrent preauricular sinus?
Hello, I underwent a preauricular sinus excision surgery for the first time in September this year.
There was no inflammation before the surgery, but I often experienced itching, which led me to decide to have it removed.
However, after the wound healed, I noticed that I still occasionally feel itching inside, which has now become less frequent, and then it stops.
Recently, I have noticed some swelling.
Is this a sign of recurrence? Could it be because the sinus was not completely excised? Can excessive fatigue trigger a recurrence?

Hello: If you frequently experience recurrent inflammation, it may be related to residual fistulas.
It is recommended to seek medical examination and treatment.
Wishing you peace and health.
Sincerely, Dr.

The recurrence of a preauricular sinus after surgical removal can be a concerning issue for patients who have undergone this procedure. A preauricular sinus is a congenital condition characterized by a small pit or cyst located near the front of the ear, which can sometimes become infected or inflamed. Surgical excision is often recommended to alleviate symptoms and prevent future complications.
In your case, it seems that you experienced some itching and discomfort after your initial surgery, which has now progressed to swelling. This could indeed be a sign of recurrence or incomplete excision of the sinus tract. It is important to understand that preauricular sinuses can sometimes have complex anatomical structures, and if the entire sinus tract is not completely removed during surgery, there is a possibility that it can recur.
The itching you experienced could be attributed to several factors, including irritation from the healing process, the presence of residual tissue, or even a mild infection. The swelling you are noticing now may indicate that there is still some inflammation or infection present, which could be related to the sinus itself or the surgical site.
Regarding your question about fatigue and its potential role in recurrence, while physical stress and fatigue can impact the immune system and overall healing, they are not direct causes of preauricular sinus recurrence. However, if your body is under stress, it may be more susceptible to infections, which could exacerbate any existing issues related to the sinus.
To address your concerns, it is crucial to follow up with your surgeon or a specialist in otolaryngology (ear, nose, and throat) for a thorough evaluation. They may recommend imaging studies or a physical examination to assess the surgical site and determine whether there is any residual sinus tissue or infection. If a recurrence is confirmed, further surgical intervention may be necessary to ensure complete removal of the sinus tract.
In terms of recovery, if you are experiencing symptoms such as swelling, pain, or persistent itching, it is essential to avoid any strenuous activities that could exacerbate these symptoms. Adequate rest and proper wound care are vital for healing. Additionally, if you notice any signs of infection, such as increased redness, warmth, or discharge from the surgical site, you should seek medical attention promptly.
In summary, while it is possible that your symptoms may indicate a recurrence of the preauricular sinus, a definitive diagnosis can only be made through a clinical evaluation. It is important to communicate your concerns with your healthcare provider, who can guide you through the next steps and provide appropriate treatment options.
