Flashes and discomfort in the fundus after retinal tear laser surgery?
Hello Doctor, I am a high myope with over 1000 degrees of myopia in both eyes and currently working in mainland China.
A month ago, I suddenly experienced flashes of light in my eyes.
After returning to Taiwan for a medical check-up, the doctor informed me that I had a retinal tear in my left eye and immediately performed laser surgery.
One day after the surgery, I still saw flashes of light, and the doctor told me this was a normal post-operative phenomenon.
I have since returned to work in mainland China and continue to see flashes of light, especially in the evening, in dim lighting, or when I am fatigued.
Recently, after seeing flashes, I have started to feel pain in the back of my eye, along with a sensation of it being slightly torn.
My vision has not worsened, and I do not see floaters, but I feel very uncomfortable.
A few days ago, I began to experience difficulty focusing and seeing some strange streaks.
The back of my left eye still feels painful.
Should I return to Taiwan for medical attention immediately, or is this a normal phenomenon? Thank you!

After undergoing laser surgery for a retinal tear, experiencing flashing lights and discomfort in the eye can be concerning, especially for someone with a history of high myopia. It’s important to understand that while some post-operative symptoms can be normal, others may warrant immediate medical attention.
Flashing lights, or photopsia, can occur after retinal surgery due to several reasons. The most common cause is the irritation of the retina during the procedure, which can lead to temporary visual disturbances. In your case, since the doctor indicated that the flashes were a normal post-operative phenomenon, it is likely that your symptoms are part of the healing process. However, the persistence of these symptoms, especially accompanied by pain and a sensation of the eye feeling "cracked," raises some concerns.
The pain you are experiencing, particularly if it is localized to the eye and is described as a feeling of pressure or discomfort, should not be ignored. While it is common to have some discomfort after surgery, significant pain, especially if it worsens or is accompanied by other visual disturbances such as loss of focus or seeing strange shapes, could indicate complications. These complications could include:
1. Retinal Detachment: Although you had a laser procedure to address a retinal tear, there is still a risk of developing a retinal detachment, especially in individuals with high myopia. Symptoms of retinal detachment can include sudden flashes of light, an increase in floaters, or a shadow or curtain over part of your vision.
2. Vitreous Detachment: In high myopia, the vitreous gel can pull away from the retina, causing flashes and discomfort. This is usually benign but can sometimes lead to more serious issues.
3. Intraocular Pressure Changes: Post-surgical inflammation or complications can lead to changes in intraocular pressure, which may cause discomfort and visual disturbances.
4. Infection or Inflammation: Any surgical procedure carries a risk of infection or inflammation, which can lead to pain and visual changes.
Given your symptoms of pain and the sensation of something being "off" in your vision, it is advisable to seek medical attention promptly. While it may be tempting to wait and see if the symptoms resolve, it is crucial to have a thorough examination by an eye care professional who can assess the situation accurately. They may perform a dilated eye exam to check the retina and vitreous for any signs of complications.
In summary, while some degree of flashing lights can be expected after laser surgery for a retinal tear, the presence of pain and other visual disturbances should not be taken lightly. It is recommended that you return to your healthcare provider for a comprehensive evaluation to rule out any serious complications. Your vision and eye health are paramount, and timely intervention can make a significant difference in outcomes.
