Post-Retinal Surgery: Visual Disturbances and Symptoms - Ophthalmology

Post-retinal surgery

Hello, doctor.
Three weeks ago, I underwent retinal laser treatment in both eyes.
Postoperatively, I occasionally see what appear to be light shadows.
Each time, I think it's something like a bug, but it turns out not to be.
This is not a flash; it’s not the lightning sensation seen at specific angles of the eye.
It occurs naturally and suddenly.
Additionally, I sometimes experience a brief sensation of flashing lights, lasting about a second, occurring once or twice a day.
My eyes also often feel dry and gritty, to the point where I need to use a tissue to wipe away tears for relief.
I would like to know if these symptoms are normal.

Dr. Gao Pilin reply Ophthalmology

Hello, the sensation of flashes is likely due to vitreous traction on the retina.
If examinations have shown no issues, you can feel reassured.
The eye discomfort may need further evaluation, as it could be related to allergic inflammation.

After undergoing retinal laser surgery, it is not uncommon to experience various visual disturbances and symptoms, which can be concerning for patients. Your description of seeing light flashes or shadows, along with feelings of dryness and discomfort, raises several important points that we can explore.

Firstly, the phenomenon of seeing light flashes or shadows, often referred to as "photopsia," can occur after retinal surgery. This can be due to several reasons, including the healing process of the retina, changes in the vitreous gel, or even residual effects from the laser treatment itself. Photopsia can manifest as brief flashes of light or the perception of shapes that are not present in the environment. While these symptoms can be alarming, they are often temporary and may diminish as the eye heals. However, it is crucial to monitor these symptoms closely, as persistent or worsening photopsia could indicate complications such as retinal detachment or other issues that require immediate attention.

The sensation of seeing lights flickering intermittently, as you described, can also be related to the same healing processes. This flickering can be a result of the retina being sensitive post-surgery or due to the changes in the vitreous body, which may tug on the retina as it begins to detach from the back of the eye. If these flickers are infrequent and not accompanied by other symptoms such as significant vision loss or a curtain-like shadow over your vision, they are likely benign. However, if you notice an increase in frequency or intensity, it is essential to consult your ophthalmologist.

Regarding the feelings of dryness and the need to wipe tears frequently, this could be attributed to several factors. Post-surgical inflammation can lead to dry eye symptoms, which may cause discomfort and a sensation of grittiness or irritation. Additionally, the use of certain medications, such as anti-inflammatory eye drops or antibiotics, can also contribute to dryness. It is advisable to use preservative-free artificial tears to help alleviate these symptoms. These drops can provide moisture and comfort to the ocular surface, especially if you find yourself needing to wipe tears often.

In summary, while the symptoms you are experiencing—light flashes, flickering lights, and dryness—can be common after retinal surgery, it is essential to maintain open communication with your eye care provider. They can perform a thorough examination to ensure that your healing process is progressing as expected and rule out any potential complications. If your symptoms persist or worsen, do not hesitate to reach out for further evaluation. Your comfort and visual health are paramount, and addressing these concerns promptly can help ensure a smoother recovery.
