Inquiry about the condition of the eye after retinal detachment surgery?

Hello Doctor,
1.
Why is it that after retinal detachment surgery, when I close my eyes to sleep at night and hear sounds (such as a neighbor closing a door, raindrops, or the sound of a motorcycle revving), I experience rapid photopic responses and an increased heart rate simultaneously, which is quite uncomfortable for me?
2.
Additionally, after the replacement of my intraocular lens with a health insurance-covered lens, I have noticed these symptoms: during the day, when I look at the street view from the third floor, I see light spots that follow the movement of my eyes.
Sometimes, when I gently move my head side to side, I can see reflections of light from the lens.
Moreover, when I move my fingers back and forth in front of my eyes, I can see the transparent optic nerve head appearing as lines that resemble nerve fibers.
However, during my follow-up visits over the past two years, the doctors have not indicated any issues.
3.
After the lens surgery, I have experienced significant discomfort in the muscles and tissues around my eyes, with pain extending deeply to my teeth and neck, making me feel extremely unwell.
I am wondering if this is a common occurrence after lens surgery, or if it might be related to the doctor's technique, as I have not recovered even after more than two years.

Thank you for your assistance in clarifying these concerns.

Hello:
1.
Increased sensitivity to sound and rapid photic response along with elevated heart rate when closing the eyes at night: After retinal detachment surgery, some individuals may experience heightened sensitivity to light and sound during the recovery period.
This could be due to the healing process of the retina or the eye post-surgery, potentially causing some discomfort.
This sensitivity may be related to the ocular condition.
It is advisable to minimize stimuli and allow the eyes to rest as much as possible.
2.
Abnormal conditions after artificial lens replacement: After the implantation of an artificial lens, some patients may experience rare phenomena, such as the perception of light spots following eye movement, reflections from the lens, and seeing the optic nerve head in front of the eyeball, as you described.
3.
Symptoms of pulling or bruising in the muscles and bones around the eyes: If symptoms of pulling or bruising in the muscles and bones surrounding the eyes occur post-surgery, they are likely unrelated to the surgery itself.
It is recommended that you consult a neurologist.

More Info

Post-retinal detachment surgery, particularly involving procedures like scleral buckle or vitrectomy, can lead to a variety of symptoms and concerns that patients may experience during their recovery. Your questions touch on several important aspects of post-operative care and symptom management, which I will address in detail.

1. Auditory Sensitivity and Eye Symptoms: It is not uncommon for patients to experience heightened sensitivity to sounds and visual stimuli after eye surgery. This can be attributed to several factors, including the stress of surgery, changes in the eye's structure, and the healing process. The quick flashes of light you describe, along with an increased heart rate when hearing sounds, could be a result of heightened awareness or anxiety related to your recovery. The brain may be more attuned to sensory inputs during this time, leading to a more pronounced reaction. If these symptoms persist or worsen, it would be advisable to discuss them with your ophthalmologist, as they may provide insights into whether these reactions are part of the normal healing process or if further evaluation is needed.

2. Visual Disturbances with Artificial Lens: After cataract surgery or lens replacement, it is common for patients to notice visual phenomena such as light spots or reflections, especially when looking at bright lights or moving their heads. These effects can be due to the interaction between the artificial lens and light, as well as the healing of the eye. The perception of "transparent lines" or reflections of the optic nerve may also be related to how your brain is processing visual information post-surgery. If these visual disturbances are bothersome or if you notice significant changes in your vision, it is crucial to follow up with your eye doctor. They can perform a thorough examination to ensure that the lens is positioned correctly and that there are no complications.

3. Physical Discomfort and Pain: The discomfort you describe in your head, neck, and facial areas after lens surgery is concerning and should not be overlooked. While some degree of discomfort can occur after any surgical procedure, persistent pain that affects your quality of life warrants further investigation. It is possible that the pain could be related to muscle strain from altered visual habits post-surgery, or it could be a result of nerve irritation or other complications. It is essential to communicate these symptoms to your healthcare provider, as they may need to assess your overall health and consider referrals to pain management specialists or physical therapists who can help address these issues.

In conclusion, while some symptoms after retinal detachment surgery and lens replacement can be expected, persistent or severe symptoms should always be evaluated by a healthcare professional. Regular follow-up appointments are crucial for monitoring your recovery and addressing any concerns. If you feel that your symptoms are not being adequately addressed, seeking a second opinion or consulting with a specialist in ocular health may provide you with the reassurance and care you need. Remember, your comfort and well-being are paramount during your recovery process.
