Experiencing the urge to defecate during sexual intercourse?

Hello doctor, I would like to ask why I feel the urge to have a bowel movement every time I engage in sexual intercourse with a male (even after using the restroom beforehand), and I also experience some pain in my vagina.
We have sufficient foreplay and I am quite lubricated.
However, after finishing, I no longer feel the urge to have a bowel movement, and I experience the same sensation even when a finger is inserted.
I have had this experience with three different partners, and my condition remains the same.
What could be the reason for this?

Miss Lin, 20~29 year old female. Ask Date: 2017/02/18

The sensation of needing to defecate during sexual intercourse can be perplexing and uncomfortable. This phenomenon can be attributed to several physiological and psychological factors that interplay during sexual activity.
Firstly, the anatomy of the pelvic region plays a significant role. The vagina and rectum are located in close proximity to each other, and during sexual activity, especially with penetration, pressure can be exerted on the rectum. This pressure can stimulate the nerves in the rectal area, leading to the sensation of needing to poop. Even if you have emptied your bowels prior to sexual activity, the physical stimulation can still trigger this urge.

Additionally, the pelvic floor muscles, which support both the bladder and the rectum, are engaged during sexual activity. If these muscles are tense or if there is a lack of relaxation, it can lead to discomfort or pain in the vaginal area. This tension can also contribute to the sensation of needing to defecate. It’s important to ensure that you are relaxed and comfortable during sexual activity, as anxiety or tension can exacerbate these sensations.

Another factor to consider is the psychological aspect of sexual activity. The brain plays a crucial role in sexual arousal and response. If you have any underlying anxiety or discomfort related to sex, it could manifest as physical sensations, including the urge to defecate. This could be particularly relevant if you have had previous experiences that were uncomfortable or if you feel any pressure to perform during sexual encounters.

Moreover, the sensation of needing to defecate could also be linked to the body's natural response to arousal. During sexual excitement, the body undergoes various physiological changes, including increased blood flow to the pelvic region, which can heighten sensitivity and lead to unusual sensations.
If you are experiencing pain during intercourse, this is another aspect that should not be overlooked. Pain during sex, known as dyspareunia, can arise from various causes, including insufficient lubrication, tension in the pelvic floor muscles, or underlying medical conditions such as vaginismus or infections. It’s crucial to address this pain, as it can affect your sexual health and overall well-being.

Given that you have experienced this sensation with multiple partners, it may be beneficial to consult with a healthcare professional, such as a gynecologist or a pelvic floor specialist. They can provide a thorough evaluation to rule out any underlying medical issues and offer guidance on how to manage these sensations during sexual activity.
In the meantime, consider focusing on relaxation techniques before and during sexual activity. Engaging in deep breathing exercises, ensuring adequate foreplay, and communicating openly with your partner about your comfort levels can help alleviate some of the discomfort and anxiety you may be experiencing.

In summary, the urge to defecate during sex can be attributed to anatomical proximity, pelvic floor muscle tension, and psychological factors. Addressing any pain and seeking professional advice can help you better understand and manage these sensations, leading to a more comfortable and enjoyable sexual experience.
