Anal itching
I apologize for the oversight in my previous question.
(I mentioned that I had a fistula surgery two weeks ago, and postoperatively I experienced itching, which was diagnosed as eczema during my follow-up visit.) Additionally, I would like to ask about my bowel movements.
I used to frequently experience diarrhea or constipation a long time ago, but it improved during college.
However, after graduating (about a year and a half ago), my irregular eating and sleeping habits caused my bowel movements to become abnormal again (diarrhea, loose stools...).
Around this time, I also began experiencing severe issues with my anal and genital areas.
It was only in the last month or two that I started actively adjusting my lifestyle and diet to alleviate the anal itching.
Since then, my bowel habits have changed to two to three times a day (soft, sometimes loose, sometimes normal), usually concentrated in the morning to noon with short intervals between them.
I would like to know if my bowel habits are considered normal or abnormal, and whether the reduction in anal itching after my last bowel movement is coincidental or if it indicates an underlying intestinal issue causing anal discomfort.

Based on your description, it seems more like irritable bowel syndrome.
It is recommended to start with a light diet and maintain a good daily routine.
Spicy, irritating, and greasy foods should be avoided.
For anal itching, please pay attention to three things: 1.
Do not scratch to prevent wound infection.
2.
Temporarily avoid spicy foods to prevent capsaicin from causing skin irritation.
3.
Avoid using soap or body wash to clean the anal area to prevent excessive removal of the skin's natural oils.
If these points are followed, improvement can generally be seen in about a week.
If symptoms persist without improvement, you may consult a physician for medication.
If there is still no improvement or if you are very concerned, a colonoscopy can be arranged.

Understanding anal itching and irregular bowel habits after surgery can be complex, especially considering the various factors that can contribute to these symptoms. Based on your description, it seems that you are experiencing a combination of anal itching, irregular bowel habits, and changes in your overall gastrointestinal function following your recent fistula surgery.
Firstly, let's address the anal itching. You mentioned that your doctor diagnosed it as eczema, which is a common skin condition that can occur in the anal area, especially after surgery. This can be due to several factors, including moisture, irritation from bowel movements, or even the use of certain hygiene products. Post-surgical changes can also lead to increased sensitivity in the area, making it more susceptible to irritation. It’s essential to keep the area clean and dry, and you might consider using barrier creams or ointments to protect the skin. If the itching persists or worsens, further evaluation by a dermatologist or your surgeon may be necessary.
Regarding your bowel habits, it’s important to note that after any abdominal or pelvic surgery, the gastrointestinal tract can take time to adjust. You mentioned that you have a history of alternating diarrhea and constipation, which can be exacerbated by stress, dietary changes, and irregular eating patterns. The fact that you are now experiencing two to three bowel movements a day, with soft and sometimes loose stools, indicates that your bowel function is still in a state of adjustment.
Typically, normal bowel habits can range from three times a day to three times a week, so your frequency falls within this range. However, the consistency of your stools is also crucial. Soft, well-formed stools are generally considered normal, while loose or watery stools may indicate an underlying issue, such as dietary intolerance or a gastrointestinal infection. Since you mentioned that your bowel movements are often clustered in the morning to midday, this could be a result of your body's natural circadian rhythm or a response to your dietary intake, especially if you consume a significant amount of fiber in the morning.
The relationship between your bowel habits and anal itching is also noteworthy. It’s not uncommon for anal discomfort to be linked to bowel movements, especially if the stools are loose or if there is any straining involved. The fact that your itching subsides after a bowel movement may suggest that the irritation is related to the bowel contents or the act of passing stool.
To improve your bowel habits and manage the itching, consider the following recommendations:
1. Dietary Adjustments: Focus on a balanced diet rich in fiber, which can help regulate bowel movements. However, if you find that high-fiber foods lead to increased gas or loose stools, you may need to adjust your intake accordingly.
2. Hydration: Ensure you are drinking enough water throughout the day, as this can help with stool consistency and overall digestive health.
3. Regularity: Try to establish a regular schedule for bowel movements. This can help train your body and may lead to more predictable patterns.
4. Hygiene Practices: After bowel movements, ensure that you clean the area gently but thoroughly. Avoid harsh soaps or wipes that may irritate the skin. Instead, consider using gentle, unscented products.
5. Consultation: If your symptoms persist or worsen, it may be beneficial to consult with a gastroenterologist. They can perform further evaluations, such as stool tests or imaging studies, to rule out any underlying gastrointestinal conditions.
In summary, while some degree of irregularity in bowel habits can be expected after surgery, persistent symptoms warrant further investigation. Addressing both the anal itching and bowel irregularities through dietary and lifestyle modifications, along with appropriate medical follow-up, can help improve your overall comfort and health.
