Elevated CA 19-9 levels?

Hello doctor, I paid out of pocket for a cancer marker test, and my CA 19-9 level has risen by over 100.
However, I currently have no discomfort or gastrointestinal issues.
I have a history of gastroenteritis and underwent an upper gastrointestinal endoscopy about five or six years ago.
I also have gynecological conditions, including a history of surgery for a breast fibroadenoma and an ovarian teratoma that has not been surgically removed.
Could these conditions be causing the increase in my CA 19-9 levels? Thank you.

Ms. Hsu, 30~39 year old female. Ask Date: 2017/03/03

Dr. Cai Anshun reply Gastroenterology and Hepatology

Hello: A high level of CA-199 does not definitively indicate the presence of cancer; it is only for reference.
However, it is generally used clinically to monitor colorectal cancer, lung cancer, bile duct cancer, and pancreatic cancer.

Elevated CA19-9 levels can be a concerning finding, especially when they rise significantly, as you mentioned with an increase of over 100. However, it is crucial to understand that CA19-9 is a tumor marker primarily associated with pancreatic cancer, but it is not specific to cancer alone. Elevated levels can also be seen in various benign conditions, including pancreatitis, liver disease, and even certain gynecological conditions.

In your case, you mentioned having a history of gastrointestinal issues, including gastritis, and gynecological conditions like a fibrous tumor in the breast and an ovarian teratoma. These conditions can potentially contribute to elevated CA19-9 levels. For instance, conditions like endometriosis or ovarian cysts can sometimes lead to increased CA19-9 levels, although this is less common.

When interpreting elevated CA19-9 levels, it is essential to consider the following:
1. Non-Specificity of CA19-9: CA19-9 is not exclusively a cancer marker. It can be elevated in various benign conditions, including pancreatitis, cholangitis, liver cirrhosis, and even in some cases of inflammatory bowel disease. Therefore, a high CA19-9 level does not automatically indicate the presence of cancer.

2. Clinical Correlation: The interpretation of CA19-9 levels should always be done in conjunction with clinical symptoms and other diagnostic tests. Since you are currently asymptomatic, this is a positive sign. However, it is still essential to follow up with your healthcare provider for further evaluation.

3. Further Testing: Given the significant elevation in your CA19-9 levels, it would be prudent to undergo further testing to determine the underlying cause. This may include imaging studies such as an abdominal ultrasound or CT scan to assess the pancreas and other abdominal organs. Additionally, your doctor may recommend further blood tests to evaluate liver function and other markers that could provide more insight into your condition.

4. Monitoring: If no immediate cause is found, your doctor may suggest monitoring your CA19-9 levels over time. This can help determine if the levels are stable, increasing, or decreasing, which can provide valuable information regarding your health status.

5. Consultation with Specialists: Given your history of gynecological issues, it may also be beneficial to consult with a gynecologist to evaluate whether your conditions could be contributing to the elevated CA19-9 levels. They may recommend imaging studies or other tests specific to your gynecological health.

In summary, while elevated CA19-9 levels can be concerning, they are not definitive for cancer diagnosis. It is essential to work closely with your healthcare provider to determine the appropriate next steps, which may include further testing and monitoring. Your history of gastrointestinal and gynecological issues may play a role in your elevated levels, and a comprehensive evaluation will help clarify the situation. Always prioritize open communication with your healthcare team to ensure you receive the best care possible.
