CA19-9 Issues

Hello, doctor.
Two to three years ago, my CA 19-9 level was less than 10.
This year, from February to May, the levels increased from 15 to 17, then 18, and finally 23.
All other markers are normal.
Does this increase indicate inflammation in the body? Should I undergo further testing? Thank you.

Mr. Wen, 30~39 year old female. Ask Date: 2023/05/31

Dr. Chen Sirong reply Oncology

Mr.
Wen / 38 years old.
CA 19-9 is a tumor marker, and elevated levels can be associated with pancreatic cancer, colorectal cancer, biliary tract cancer, and gastric cancer.
Occasionally, conditions like cholecystitis and acute pancreatitis can also result in increased levels.
Some benign diseases, such as endometriosis, liver cysts, and renal cysts, may also cause elevated CA 19-9 levels.
Therefore, a CA 19-9 level above the standard does not necessarily indicate the presence of cancer.
Generally, the increase in CA 19-9 may be related to diseases categorized as "cystic," "inflammatory," or "tumorous." For further evaluation, the following can be arranged: 1.
Monitor CA 19-9 levels every three months, 2.
Comprehensive health check-up, 3.
Targeted examinations for the aforementioned areas.
Wishing you good health and well-being.
Taoyuan Hospital, Dr.
Chen Sih-Rong.

More Info

CA19-9 is a tumor marker primarily associated with pancreatic cancer, but it can also be elevated in other conditions, including various gastrointestinal cancers, liver diseases, and even benign conditions such as pancreatitis and cholangitis. The normal range for CA19-9 is typically considered to be less than 37 U/mL. Your recent results show a gradual increase from 15 to 23 U/mL over a few months, which raises some important considerations regarding your health.

Firstly, it's essential to understand that a slight elevation in CA19-9 levels does not automatically indicate the presence of cancer. Many factors can contribute to elevated CA19-9 levels, including inflammation, benign tumors, and other non-cancerous conditions. For instance, conditions like pancreatitis, liver cirrhosis, and even certain infections can lead to increased CA19-9 levels. In your case, the gradual rise from 15 to 23 U/mL, while notable, is still below the threshold that typically raises significant concern for malignancy, especially since other tumor markers are normal.

Given that your CA19-9 levels have increased but remain relatively low, it may not be immediately alarming, but it does warrant further investigation to rule out any underlying issues. Here are some recommendations based on your situation:
1. Regular Monitoring: It is advisable to continue monitoring your CA19-9 levels. A follow-up test in a few months can help determine if the trend continues upward, stabilizes, or decreases. This can provide more context for your health status.

2. Comprehensive Health Check: A thorough health evaluation, including imaging studies such as an abdominal ultrasound or CT scan, may be beneficial. These tests can help visualize the pancreas, liver, and biliary system to check for any abnormalities that could explain the elevated CA19-9 levels.

3. Consultation with Specialists: Depending on the results of your imaging studies, you may want to consult with a gastroenterologist or an oncologist. They can provide more specialized insights and determine if further diagnostic procedures, such as endoscopic ultrasound or biopsy, are necessary.

4. Consider Other Symptoms: Pay attention to any other symptoms you may experience, such as abdominal pain, weight loss, changes in appetite, or digestive issues. These symptoms can provide additional clues about your health and should be discussed with your healthcare provider.

5. Lifestyle Factors: While your CA19-9 levels are a concern, it’s also important to consider lifestyle factors such as diet, exercise, and stress management, which can impact overall health and inflammation levels in the body.

In summary, while your CA19-9 levels have shown a slight increase, they are still within a range that may not indicate a serious condition. However, it is crucial to remain vigilant and proactive about your health. Regular monitoring, further diagnostic testing, and consultations with healthcare professionals will help ensure that any potential issues are identified and addressed promptly. Remember, early detection and intervention are key in managing health concerns effectively.
