CA19-9 Issues
Hello, doctor.
Two years ago, my CA 19-9 level was 5.
This year, from February to May, the levels increased from 15 to 17, 18, and then 23.
All other markers are normal, abdominal ultrasound is fine, and the gastroscopy showed superficial gastritis.
Does this increase indicate inflammation in the body? Should I undergo further tests? Thank you.

Currently, the health insurance can only provide follow-up.
If you are still concerned, you may consider paying out of pocket for an abdominal CT scan or MRI.

CA 19-9, or carbohydrate antigen 19-9, is a tumor marker primarily associated with pancreatic cancer, but it can also be elevated in various other conditions, including benign diseases. Understanding the implications of rising CA 19-9 levels is crucial for assessing your health, especially given your recent test results.
In your case, the CA 19-9 levels have shown a gradual increase from 15 to 23 over a few months. While this increase may raise concerns, it is essential to interpret these numbers in the context of your overall health and any accompanying symptoms. A CA 19-9 level of 5 is considered normal, and levels below 37 U/mL are generally regarded as within the normal range. However, levels above this threshold can indicate various conditions, not just malignancies.
The fact that your other health indicators are normal, along with the results from your abdominal ultrasound and gastroscopy, suggests that there may not be an immediate cause for alarm. The presence of superficial gastritis, while it can cause discomfort, is not typically associated with significantly elevated CA 19-9 levels. Instead, your rising numbers could be attributed to several factors:
1. Benign Conditions: Conditions such as pancreatitis, cholangitis, or even liver diseases can lead to elevated CA 19-9 levels. If you have a history of any of these conditions, they could explain the increase.
2. Inflammation: As you suggested, inflammation in the body can lead to elevated CA 19-9 levels. This could be due to an acute or chronic inflammatory process, which may not necessarily indicate cancer.
3. Physiological Variability: Individual variations in CA 19-9 levels can occur due to factors such as diet, hydration status, and even genetic predispositions. Some individuals may have naturally higher levels without any underlying pathology.
Given the gradual increase in your CA 19-9 levels, it is advisable to continue monitoring these levels over time. Regular follow-up tests can help determine if this trend continues or stabilizes. If the levels rise significantly or if you develop new symptoms, further investigation may be warranted.
In terms of next steps, while your current health assessments are reassuring, you might consider discussing with your healthcare provider the possibility of additional imaging studies, such as a CT scan or MRI, to rule out any underlying issues that may not have been detected in previous evaluations. These imaging modalities can provide a more comprehensive view of your abdominal organs and help identify any potential abnormalities.
In summary, while rising CA 19-9 levels can be concerning, they do not automatically indicate cancer, especially in the absence of other alarming symptoms or findings. Continuous monitoring, along with a thorough clinical evaluation, will be key in understanding the significance of your CA 19-9 levels and ensuring your overall health remains stable. Always consult with your healthcare provider for personalized advice and recommendations based on your specific situation.
