Glans inflammation

Hello doctor, about a month and a half ago, I noticed some redness on my glans.
At that time, I was washing with very hot water, and my skin was quite dry.
I found that there was some bleeding in the affected area, and it started to hurt and had some pus.
I quickly went to a small clinic.
Initially, the doctor there prescribed a white ointment and some oral anti-inflammatory medication.
After using it for a while, I felt a bit better, but I noticed that an area that wasn't inflamed before suddenly became inflamed.
By that time, I had been applying the medication for about nine days.
Later, I went to a larger hospital, where the doctor prescribed Mecon Cream, Doxycycline (an antibiotic), and Allegra (an allergy medication, as he suspected I might have an allergy).
So far, I haven't seen much improvement.

I researched online and found that balanitis can be caused by two things: a Candida infection or local trauma, friction, irritants like contraceptives, soap, and cleaning agents.
Could it be that my condition is not a Candida infection but rather caused by washing with water that was too hot and the dry weather? Additionally, I would like to ask if it's okay to take a bath.
If I bathe with my girlfriend, is there a risk of transmitting anything to her? Thank you.

Hello: First, keep the area clean and dry.
If you have a foreskin, make sure to pat dry with toilet paper after urination; do not use soap or body wash, just rinse with warm water, avoiding hot water.
Taking a bath is not recommended; a shower is fine.
If there is no improvement in 3-4 days (it doesn't have to be completely healed), you should see a doctor.
(Avoid alcohol and spicy foods.) If you have engaged in unsafe sexual practices, inform your doctor and get a blood test.
A diagnosis of candidiasis should be made by a physician; however, candidiasis is often difficult to fully treat and can recur if not managed properly.
Be cautious with the use of any topical medications as prescribed by your doctor.
Regarding whether bathing with your girlfriend could transmit the infection to her: it will not (but it is advised to use condoms during sexual activity).

Understanding and managing inflammation of the glans, or glansitis, can be a complex issue, as it often involves various underlying causes and requires appropriate treatment strategies. Based on your description, it seems you are experiencing symptoms consistent with glansitis, including redness, dryness, bleeding, pain, and discharge. Let's break down the potential causes, treatment options, and considerations for your situation.

Causes of Glansitis
1. Infections:
- Candida (Yeast) Infections: This is a common cause of glansitis, particularly in individuals with diabetes or those who are immunocompromised. Symptoms often include itching, redness, and a thick, white discharge.

- Bacterial Infections: Conditions like balanitis can arise from bacterial infections, which may be exacerbated by poor hygiene or skin irritation.

- Sexually Transmitted Infections (STIs): Certain STIs can also lead to inflammation of the glans.

2. Irritation and Allergies:
- Chemical Irritants: Soaps, lotions, or other personal care products can cause allergic reactions or irritation.
- Physical Irritation: Friction from sexual activity, tight clothing, or excessive washing with hot water can lead to inflammation.

3. Skin Conditions:
- Conditions such as psoriasis or eczema can also affect the genital area, leading to inflammation and discomfort.

Treatment Options
1. Topical Treatments:
- Antifungal Creams: If a yeast infection is suspected, antifungal creams like clotrimazole or miconazole may be prescribed.

- Antibiotic Ointments: For bacterial infections, topical antibiotics can help reduce inflammation and clear the infection.

- Corticosteroid Creams: These may be used to reduce inflammation and itching, especially if an allergic reaction is suspected.

2. Oral Medications:
- Antibiotics: If a bacterial infection is confirmed, oral antibiotics may be necessary.

- Antihistamines: If allergies are suspected, antihistamines can help alleviate symptoms.

3. Hygiene Practices:
- Maintain good hygiene by gently cleaning the area with mild soap and water. Avoid harsh soaps or chemicals.

- Avoid hot water baths or showers, as they can exacerbate dryness and irritation.

Lifestyle Considerations
- Bathing: While bathing can be soothing, it is essential to avoid hot water, as it can worsen inflammation. If you choose to bathe, ensure the water is lukewarm and avoid using any irritating soaps or bath products.

- Sexual Activity: It is advisable to avoid sexual activity until the inflammation has resolved to prevent further irritation and potential transmission of infections. If you are concerned about transmission to your partner, it is best to consult with a healthcare provider for personalized advice.

Follow-Up
Given that you have been experiencing symptoms for over two weeks and have not seen significant improvement, it is crucial to follow up with your healthcare provider. They may consider further diagnostic tests, such as swabs or cultures, to identify the specific cause of your symptoms and adjust your treatment plan accordingly.

In summary, glansitis can arise from various causes, including infections, irritants, and skin conditions. Proper diagnosis and treatment are essential for effective management. If symptoms persist or worsen, seeking further medical evaluation is necessary to ensure appropriate care and recovery.
