Glans and Prepuce Inflammation - Corticosteroids
Hello Doctor, I would like to ask a question.
A few days ago, I visited a clinic due to inflammation, and the physician prescribed me a tube of clotrimazole.
I applied it to the glans and the opening of the foreskin.
The next evening while showering, I noticed that my foreskin had swollen completely, making it impossible for me to expose the glans for cleaning.
Could this be related to the ointment? How can I reduce the swelling?

Dr. Du Shixi reply Urology
Hello: It is likely inflammation causing the swelling.
If there is no improvement after 1-2 days, you should seek medical attention.
It is important to keep the area clean and dry, and avoid tight clothing.
Wishing you good health.

Understanding steroid treatment for glans and prepuce inflammation involves recognizing the underlying causes of the inflammation, the role of steroids in treatment, and the potential side effects of such treatments.
In your case, it seems that you were prescribed a topical steroid, which is often used to reduce inflammation and alleviate symptoms associated with conditions like balanitis (inflammation of the glans) or posthitis (inflammation of the prepuce). Steroids work by suppressing the immune response and reducing inflammation, which can help relieve symptoms such as swelling, redness, and discomfort.
However, the reaction you experienced—swelling of the prepuce to the point where the glans is not visible—could be due to several factors. It is essential to consider the following:
1. Allergic Reaction or Sensitivity: Some individuals may have a sensitivity or allergic reaction to certain ingredients in topical medications, including steroids. This could lead to increased inflammation rather than relief.
2. Overuse of Steroids: While topical steroids can be effective, overuse or inappropriate application can lead to skin thinning, increased susceptibility to infections, or paradoxical worsening of inflammation.
3. Underlying Infection: If there is an underlying infection (bacterial, fungal, or viral), the use of steroids may exacerbate the condition by suppressing the local immune response, allowing the infection to worsen.
4. Improper Application: If the medication was applied too liberally or not as directed, it could lead to localized irritation or swelling.
To address the swelling and discomfort, here are some steps you can take:
- Discontinue Use: Stop using the steroid cream until you consult your healthcare provider. Continuing to use it may worsen the condition.
- Consult Your Doctor: It is crucial to follow up with your healthcare provider as soon as possible. They may need to evaluate the area to determine the cause of the swelling and recommend appropriate treatment.
- Cold Compress: Applying a cold compress to the affected area may help reduce swelling and discomfort temporarily.
- Gentle Hygiene: Maintain gentle hygiene practices. Avoid harsh soaps or irritants in the area until the swelling subsides.
- Alternative Treatments: Depending on the diagnosis, your doctor may prescribe an alternative treatment, such as antifungal or antibacterial medications if an infection is present.
In summary, while steroids can be effective in treating inflammation of the glans and prepuce, they must be used cautiously and under medical supervision. The swelling you are experiencing may not be a direct result of the steroid itself but could indicate an allergic reaction, improper use, or an underlying infection. Always consult your healthcare provider for personalized advice and treatment options tailored to your specific condition.
