Redness and tearing of the foreskin?

Hello Dr.
Du, I noticed that since the day before yesterday, half of my foreskin has been red and somewhat painful.
I used a topical ointment yesterday and this morning, but just now while showering, I found that the originally red area has turned into broken skin.
What could be the possible causes of this condition? Should I continue using the ointment or should I keep the area dry and observe for a day or two? Thank you for your assistance.

Hello: You should not use a small nurse there anymore, even if there is no broken skin in the future.
Currently, since there is broken skin, you need to seek medical attention.
What you can do is to keep the area clean and dry, and avoid soaking it in water.
Additionally, if there has been any unsafe sexual behavior, please inform your doctor and do not hide it, as it will help the doctor monitor your condition.
If there has been no unsafe sexual behavior, then it is relatively safer.
Thank you for your inquiry.

Redness and skin breakage in the foreskin can be concerning and may arise from various causes. Understanding these causes and how to care for the affected area is essential for effective management and recovery.

Possible Causes
1. Irritation or Friction: One of the most common reasons for redness and skin breakage in the foreskin is irritation due to friction. This can occur from activities such as sexual intercourse, masturbation, or even tight clothing. If the skin is sensitive, it may become inflamed and break down.

2. Infections: Infections, including yeast infections or bacterial infections, can lead to redness, swelling, and skin breakdown. Conditions like balanitis (inflammation of the glans) can also cause similar symptoms. If there is a foul odor, discharge, or significant pain, it may indicate an infection that requires medical attention.

3. Dermatitis: Contact dermatitis, which can be caused by soaps, lotions, or other products that come into contact with the genital area, may lead to redness and irritation. Allergic reactions to certain materials, such as latex in condoms, can also contribute to these symptoms.

4. Poor Hygiene: Inadequate cleaning of the genital area can lead to the accumulation of smegma, which can irritate the skin and cause inflammation.

5. Skin Conditions: Conditions such as psoriasis or eczema can affect the genital area, leading to redness and skin changes. These conditions may require specific treatments to manage effectively.

Care and Management
1. Maintain Hygiene: Gently clean the area with mild, unscented soap and water. Avoid harsh soaps or products that can further irritate the skin. After washing, ensure the area is thoroughly dried to prevent moisture buildup, which can exacerbate irritation.

2. Avoid Irritants: If you suspect that a product (such as soap, lotion, or laundry detergent) may be causing irritation, discontinue its use. Opt for hypoallergenic products designed for sensitive skin.

3. Keep the Area Dry: If the skin is broken, it's crucial to keep the area dry to promote healing. Avoid tight clothing that may cause friction and irritation. Loose-fitting underwear can help minimize discomfort.

4. Topical Treatments: If you are using a topical treatment like "小護士" (a common ointment), it may help soothe the area. However, if the skin is broken, it’s essential to consult a healthcare professional before continuing to apply any product. Some ointments may not be suitable for open wounds and could potentially cause irritation or infection.

5. Observation: If the redness and skin breakage are mild and not accompanied by severe pain, discharge, or other concerning symptoms, you may choose to observe the area for a couple of days. Monitor for any changes, such as increased redness, swelling, or the development of pus, which would necessitate medical evaluation.

6. Consult a Healthcare Professional: If symptoms persist, worsen, or if you experience significant pain, it is crucial to seek medical advice. A healthcare provider can assess the area, provide a proper diagnosis, and recommend appropriate treatment, which may include antifungal or antibacterial medications if an infection is present.

Conclusion
Redness and skin breakage in the foreskin can result from various factors, including irritation, infections, and skin conditions. Proper hygiene, avoidance of irritants, and careful observation are essential for managing the symptoms. However, if the condition does not improve or worsens, consulting a healthcare professional is vital to ensure appropriate care and treatment.

Hello: In response to your question, a tear in the foreskin is generally caused by a tight foreskin ring or a tear in the frenulum. The wound typically heals within a week, but if the tear is due to the aforementioned reasons, circumcision may need to be considered.
